CLASS 1L or MNOR - N-GtkCe to Ccinpiy) <br />Item # <br />Remarks <br />205 <br />CCR 2712(i) Failure to submit and have current LIST Monitoring Plan readily accessible at the facility. <br />OBSERVATION: Ar uprated copy of the diesel monitoring plans were nm available for inspect -ion- The monitoring <br />plan lists the VR -409 as the annular sensor for the diesel tank, but it is utilizing the VR -3413 annular sensor. <br />REGULATION GUIDANCE: A copy of these monitoring plans shall be accessible on site at all trnes. <br />CORRECTIVE ACTION: Immediately update the monitoring plan and retain a copy on site. <br />This is a repeat violation, 'Class 11. <br />208 <br />CCR 2716(a -e) Failure to comply ;kith the designated operator inspection requirements. <br />OBSERVAT !ON: —ine designated operator failed to check and note all required items on the visual -inspection <br />reports. The incorrect and missing information included the following from the inspection reports: <br />- UDC 9/10 was not included in the visual inspection reports. <br />- DO reports did not indicate that sumps with fuel alarms were visually inspected. <br />- Secondary Containment Test Data is incorrect, the correct date is 01/1512021. <br />- DO indicated that hav;na a qualified technician observe the sumps with fuel alarms was not applicable vrhen there <br />were fuel alarms. <br />REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br />tesbng and maintenance for th:e UST system have been completea and shall check a,', required items on the report. <br />CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br />all of the required info oration on 'the reports. <br />OBSERVATPON: The designated opera-ci- failed to document ali the alarms from the attached alarm history or t <br />I <br />March, May and December designated operator inspection reports and failed to check that they were responded to <br />appropriately.
