Operation and employee training records shall be maintained for 3 years and made available for inspection. A written Infection <br /> Prevention and Control Plan (IPCP) shall be maintained, followed and updated. Correct or remove the wet contact time from the <br /> IPCP. Provide EHD with an updated copy of the IPCP. <br /> 37. HSC 119312- Permits obtained and available <br /> OBSERVATIONS: <br /> EHD verified with facility owner that there are currently four practitioners at the facility. EHD records indicate there are ten active <br /> practitioners at the facility. <br /> CORRECTIVE ACTIONS: <br /> A body art facility shall notify the LEA in writing within 30 days of resignation, termination of a practitioner. A new hire or <br /> apprentice must be registered with the EHD prior to performing body art at the facility. Ensure the EHD is notified, in writing, of all <br /> practitioners that are no longer practicing at the facility within 30 days. <br /> Correct all violations and provide evidence of corrections to the EHD <br /> by July 29, 2024. <br /> Notes: <br /> 1. Inspection report emailed to facility owner, Michael Carter, at lostdreamstattooandpiercing@gmail.com. <br /> 2. The lot numbers for the POINT piercing needles did not match the sterilization certificate(s) provided. Practitioners will <br /> continue to use the Metal Mafia needles until the correct sterilization certificates are provided to the EHD. <br /> 3. Sharps waste is picked up by Stericycle. Last pick up was on March 27, 2024. <br /> 4. There are currently two piercers and two tattoo artists at the facility. <br /> 5. Per facility owner, the canvass paintings will be coated with epoxy resin coating to waterproof the paintings. This method is <br /> approved by the EHD. The paintings may be hung in the procedure area after the paintings are coated. <br /> 6. Sterilization certificates were provided for the Metal Mafia piercing needles with the lot number 231201 and the Quelle Tatt <br /> needles with the lot number 2023/08. <br /> 7. Facility owner is contemplating using electronic client records in the future. EHD must approve the forms prior to usage. <br /> 8. Facility owner is to contact EHD for a consultation prior to installing a sink in the piercing room. <br /> Reinspection on/about: A reinspection fee of$172 per hour may be charged.
