The wet contact time indicated on the bottle of Madacide is 5 <br /> minutes. <br /> CORRECTIVE ACTIONS: <br /> Disinfectant shall be used according to the manufacturer's instructions. Train practitioners on the correct wet contact time for all <br /> disinfectants used at the facility. Provide training record to the EHD. <br /> 32. HSC 119314-Floors and walls clean and in good repair, adequate light <br /> OBSERVATIONS: <br /> Holes observed on the walls of David Javier and Sonny"Jerome" Brown procedure area. <br /> CORRECTIVE ACTIONS: <br /> A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. Repair <br /> all holes and provide EHD photographic evidence of correction. <br /> 33. HSC 119314-Workstation, surfaces, including chairs, armrests, etc. in good repair <br /> OBSERVATIONS: <br /> 1.Absorbent canvass paintings observed in various areas of the shop. —Corrected on site. <br /> 2. The procedure table in the piercing room was torn. <br /> 3. Some of the decorations in the piercing room and the curtain to the entrance of the piercing room are absorbent. <br /> CORRECTIVE ACTIONS: <br /> 1. All surfaces and objects in the procedure area, including chairs, armrests, tables, countertops, and trays, shall be in good <br /> repair and capable of being disinfected. All counter surfaces and service trays in the procedure area shall have a smooth, <br /> durable, and nonabsorbent finish. <br /> 2. Post Ava Reyes' current permit and provide <br /> photographic evidence to the EHD. <br /> 35. HSC 119313 -Operation and employee training records present <br /> OBSERVATIONS: <br /> 1. The facility uses Sani-cloth disinfectant and it is not mentioned on the IPCP. <br /> 2. The disinfectant wet contact time stated on the IPCP is 3 minutes. The wet contact time mentioned on the manufacturer's <br /> instruction for Madacide is 5 minutes. <br /> CORRECTIVE ACTIONS: <br /> 1. Operation and employee training records shall be maintained for 3 years and made available for inspection. A written Infection <br /> Prevention and Control Plan (IPCP) shall be maintained, followed and updated.
