The wet contact time indicated on the bottle of Madacide is 5 minutes. CORRECTIVE ACTIONS: Disinfectant shall be used according to the manufacturer's instructions. Train practitioners on the correct wet contact time for all disinfectants used at the facility. Provide training record to the EHD. 32. HSC 119314-Floors and walls clean and in good repair, adequate light OBSERVATIONS: Holes observed on the walls of David Javier and Sonny"Jerome" Brown procedure area. CORRECTIVE ACTIONS: A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. Repair all holes and provide EHD photographic evidence of correction. 33. HSC 119314-Workstation, surfaces, including chairs, armrests, etc. in good repair OBSERVATIONS: 1.Absorbent canvass paintings observed in various areas of the shop. —Corrected on site. 2. The procedure table in the piercing room was torn. 3. Some of the decorations in the piercing room and the curtain to the entrance of the piercing room are absorbent. CORRECTIVE ACTIONS: 1. All surfaces and objects in the procedure area, including chairs, armrests, tables, countertops, and trays, shall be in good repair and capable of being disinfected. All counter surfaces and service trays in the procedure area shall have a smooth, durable, and nonabsorbent finish. 2. Post Ava Reyes' current permit and provide photographic evidence to the EHD. 35. HSC 119313 -Operation and employee training records present OBSERVATIONS: 1. The facility uses Sani-cloth disinfectant and it is not mentioned on the IPCP. 2. The disinfectant wet contact time stated on the IPCP is 3 minutes. The wet contact time mentioned on the manufacturer's instruction for Madacide is 5 minutes. CORRECTIVE ACTIONS: 1. Operation and employee training records shall be maintained for 3 years and made available for inspection. A written Infection Prevention and Control Plan (IPCP) shall be maintained, followed and updated.
