Lima Ranch 39-344 <br /> 13436 N Thornton Road Page 2 <br /> Lodi, CA 95242 <br /> Since the housing accommodations noted above include a duplex, you would now be required to <br /> obtain an annual employee housing Permit to Operate. <br /> As these Permit to Operate provisions are a new and added responsibility, EHD staff will make every <br /> effort to answer any questions and provide assistance to make this process as easy and <br /> straightforward as possible. <br /> The following information on employee housing requirements is provided for your assistance: <br /> • Employee Housing Requirements - The requirements for employee housing can be found in <br /> the Employee Housing Act, California Health and Safety Code, Section 17000 et seq., and the <br /> California Code of Regulations, Title 25, Division 1, Chapter 1, Subchapter 3. Additionally, the <br /> "Employee Housing Facility Inspection Information Booklet for Owners, Operators, and <br /> Residents," and the "Guide to State and Federal Requirements for Employee/Migrant <br /> Housing,"are also enclosed for your guidance. <br /> • Annual Permit to Operate Fees - The EHD permit fees for calendar year 2014 are $35.00 <br /> per facility, plus $12.00 for each employee. An application for an EHD Permit to Operate for <br /> calendar year 2014 will be mailed to you in November 2013. The application must be <br /> completed and returned to EHD with the appropriate permit fees. <br /> • Annual Employee Housing Inspections - The Employee Housing Act requires employee <br /> housing accommodations to be inspected at least annually, before the Permit to Operate can <br /> be issued. Beginning in January 2014, EHD staff will contact you to schedule a time for this <br /> inspection. Please use the guidance documents enclosed with this letter to review the <br /> minimum requirements for employee housing. These are the items, with the exception of those <br /> relating specifical y to dormitory-style housing, that will be checked during the inspection. <br /> If the inspection identifies any areas that don't meet the minimum State requirements, please <br /> be assured that the EHD will furnish information on how to correct the issues and provide <br /> sufficient time for the corrections to be made. Once completed, EHD staff will perform a re- <br /> inspection to document that the corrections have been accomplished. <br /> • Permit to Operate - After the inspection process is complete and all minimum State <br /> requirements have been met, an EHD Permit to Operate will be issued for your facility. <br /> Thank you for your assistance with this matter.
