16. Self Monitoring: The owner/ope shall submit the results of all self-monitoring pr 1s <br /> to the Enforcement Agency within 30 days of the end of the reporting period. <br /> (for example,I st quarter=January—March,the report is due by April 30,etc. Information required <br /> on an annual basis shall be submitted with the 4th quarter monitoring report,unless otherwise stated) <br /> Program Reporting Frequency <br /> a. Log of Special Occurrences(i.e.tires,explosions,accidents,inadvertent a. ANNUALLY TO LEA <br /> acceptance of hazardous waste,etc...)must be kept on site and submitted to <br /> LEA annually. <br /> b. An employee training log with course descriptions,dates of training,etc...shall b. KEEP ON SITE&AVAILABLE <br /> be maintained and kept current. UPON REQUEST OF LEA <br /> c. A record of weather conditions shall be maintained with operation records c. KEEP ON SITE&AVAILABLE <br /> pursuant to 14 CCR Section 17869. UPON REQUEST OF LEA <br /> d. A report of quantities and types of waste received and product removed from d. QUARTERLY TO LEA <br /> the facility shall be provided to the LEA on a quarterly basis(showing <br /> monthly breakdowns). The records shall indicate the quantities in tons/cubic <br /> yards and include weight and record data. Records shall be submitted to the <br /> LEA on the 15d day of the subsequent month. <br /> e. Temperature of the active compost shall be monitored and recorded as e. KEEP ON SITE&AVAILABLE <br /> specified in Title 14 CCR, Section 17868.3. Records shall be kept for kinds UPON REQUEST OF LEA <br /> and amounts of additives/amendments used in composting process. Whatever <br /> composting method used,all records pertinent to the particular operation are <br /> required to be kept current on site as per 14 CCR Section 17869. <br /> f. In addition to storm waster monitoring records,a log shall be kept for leachate f. QUARTERLY TO LEA& <br /> monitoring. REGIONAL WATER QUALITY <br /> CONTRLOL BOARD(RWQCB) <br /> g. One sample per 5000 cubic yards of finished compost must be taken and g• MONTHLY TO LEA <br /> analyzed at a Department of Health Services Approved Laboratory. Samples <br /> shall be analyzed for constituents required pursuant to 14 CCR,Section <br /> 17868.1. <br /> h. Pathogen reduction criteria must be documented in a log. The log must record h. MONTHLY TO LEA <br /> start date,temperatures,turnings,and watering of windrows. All feedstock <br /> piles must be monitored for temperature prior to being incorporated into <br /> active compost windrows. <br /> i. Submit an annual report to the LEA by January 31,or each calendar year i. ANNUALLY TO LEA <br /> summarizing all reporting requirements. <br /> j. The types and quantities(in tons)of waste,including separated or commingled j• MONTHLY TO LEA <br /> recyclables,entering the facility per day. <br /> k. The number and types of vehicles using the facility per day. k. KEEP ON SITE&AVAILABLE <br /> UPON REQUEST OF LEA <br /> 1. Results of the hazardous waste load checking program,including the 1. KEEP ON SITE&AVAILABLE <br /> quantities and types of hazardous wastes,medical wastes or otherwise UPON REQUEST OF LEA <br /> prohibited wastes found in the waste stream and the disposition of these <br /> materials. <br /> M.
