Determine from the <br /> doctor if the employee should be treated at the doctor's office, or sent to the company <br /> designated medical facility for treatment. <br /> NOTE: If an injury occurs during non-business hours, ask the doctor's answering service <br /> to contact the physician on call. Confirm where the patient is to be sent and if a doctor will <br /> meet the patient at that iocadon. Provide transportation for the employee. <br /> Write up a supervisor's report of injury as soon as reasonably possible. <br /> GENERAL INFORMATION: <br /> 1. It is important that we get written reports on all accidents and any near misses. Keep <br /> your supervisor informed of unsafe conditions and unsafe acts. It is only when the <br /> Company knows where and why accidents are happening that it will be able to take <br /> appropriate measures to prevent them from reoccurring in the future. <br /> 2. Report all injuries to your supervisor immediately. The Company's Worker's <br /> } Compensation Insurance may not accept claims unless there was a timely report of the <br /> injury when it first occurred. <br /> 3. You are to seek medical attention for on-the-job injuries through the regular procedure <br /> established by the Company: <br /> A. The Company maintains first aid supplies to take care of minor injuries.' <br /> B. After seeking first aid treatment, notify your supervisor. In case of a serious <br /> accident or illness, contact any available supervisor, manager or trained first aid <br /> person. <br /> C. If you require medical attention beyond first aid offered by the Company, <br /> arrangements will be made for you to be treated by a company designated <br /> physician. After treatment by the physician, if you are not satisfied, contact the <br /> facility manager to explain your dissatisfaction and request a change of <br /> physicians. <br /> D. After you have been seen by a physician, you must return to the Company offices <br /> with the doctor's report. Before you will be scheduled to return to work you <br /> must have the physician's full and complete release to return to work. If not, <br /> you may be granted a temporary leave of absence. <br /> 4. Injuries or illnesses which are riot caused by the job, will be treated by the Company <br /> on an emergency basis only. These cases should be taken care of by the employee, if <br /> at all possible. <br /> 16 <br />
