Please be <br />aware that all EHD staff time associated with a reinspection will be billed at our current hourly rate ($156). <br />This is a repeat violation (Class 2) Violation. <br />53 <br />CA Health and Safety Code 25292.2; CCR 2711(a)(11) 2010007 - Submitted/maintained current Cert of Financial <br />Responsibility/mech of financial assurance (TCR 11). <br />OBSERVATION: Financial responsibility documents have not been submitted to the EHD. The most recent financial <br />responsibility documents in CERS are dated 11/8/2021. <br />REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, and upload the required documents. <br />This is a repeat violation (Class 2) Violation. <br />56 <br />CA Code of Regulations 2716(a -e) 2030013 - DO inspections completed at least every 30 days and meet all <br />requirements (TCR 12). <br />1) OBSERVATION: The designated operator failed to check and note the correct test dates for the Secondary <br />Containment Test on the 8/25/2023 designated operator inspection report. A date was not listed on this report. The <br />correct date for Secondary Containment Test is 2/15/2023 <br />REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br />maintenance for the UST system have been completed and document the dates they were done. <br />CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the <br />required information on the reports. <br />------------------------------------------------------------------------- <br />2) OBSERVATION: Designated operator reports were not signed within the required time frame. <br />-The 9/22/2023 designated operator report was signed by the owner/operator on 9/29/2023 <br />- The 5/31/2024 designated operator report was not signed or dated by the owner/operator <br />REGULATORY GUIDANCE: Within 48 hours of the completion of the designated UST operator visual inspection, the <br />designated operator shall sign and provide the owner or operator with a copy of the inspection report.
