E 0 FIVE Health and Safely Requirements <br /> o Visible signs of chemical permeation such as swelling,discoloration, stiffness or brittleness <br /> o Cracks or any signs of puncture or abrasion <br /> Any reusable garments exhibiting any such characteristics will be discarded. i <br /> The duration of work tasks in which personnel use PPE ensembles that include chemical protective <br /> clothing(including uncoated Tyvek®)will be established by the SSO.Variables to be considered include <br /> ambient temperature and other weather conditions,the capacity of individual personnel to work in the <br /> required level of PPE in heat and cold, and the limitations of specific PPE ensembles. The recommended <br /> rest breaks are as follows: <br /> • Fifteen minutes midway between shift startup and lunch <br /> • Lunch break(30-60 minutes) <br /> • Fifteen minutes midway between lunch and shift end <br /> Rest breaks are to be taken in the support zone or other clean area after personnel have completed the <br /> decontamination process,including soap and water wash of hands and face. <br /> Personnel will be monitored and the work schedule adjusted as necessary during periods of elevated <br /> ambient temperatures above 85°F or humidity greater than 50%.Activities requiring strenuous labor will, <br /> whenever feasible,be scheduled during morning hours during hot weather.To control heat stress, <br /> personnel will follow the specific heat-related health and safety procedures described in SMS 018 (Heat <br /> Stress). <br /> 5.7 SITE CONTROL MEASURES <br /> At the work site, site control measures must be enforced to prevent unauthorized personnel into the <br /> designated work area. The URS SHSO should clearly define and identify the work zones based on <br /> several area-specific criteria,including the physical features of the work area,traffic activity,wind <br /> direction,and road activity. <br /> The SHSO should identify the work area during the mobilization of the equipment and materials at each <br /> planned location. Once work commences,no outside personnel are allowed into the work area without <br /> appropriate gear and proof of health and safety training. <br /> 5.8 TRENCHING AND EXCAVATION SAFETY <br /> For this project work,URS personnel are permitted to enter a trench or excavation deeper than four feet. <br /> All the provisions of URS SMS 13 (Excavation Safety)must be provided prior to entry by the contractor <br /> who created the excavation including: <br /> UMH:\WPROCESS\25532\WORLDENTERPRISE\WELL DESTRUCTION\ATTACHMENT C WORLD ENTERPRISES HSP TG EDITS.DOC\18-NOV-11\\ 5-4 <br />
