SMONFIVE Health and Safety Requirements <br /> 5.5 ACTION LEVELS <br /> For excavation operations,a combustible gas indicator(CGI)with a remote sensing head should be used. <br /> The sensing head should be attached to the excavator arm near the bucket,and the cable run back along <br /> the arm to the CGI located in the excavator cab.This will permit the operator to be alerted to hazardous <br /> situations without requiring monitoring personnel to stand at the working face. (Table 4-3) <br /> Fire suppression equipment(Two 2AlOB:C fire extinguishers or fire hoses)is to be present at all times <br /> during site operations in areas where a fire potential exists. <br /> 5.6 PERSONEL PROTECTIVE EQUIPMENT <br /> Minimum Personal Protective Equipment(PPE)for site personnel includes: <br /> • Hardhat; <br /> • Safety glasses with side shields(or impact resistant goggles); <br /> • Steel-toed boots or chemical-resistant steel-toed boots; <br /> • Ear protection in vicinity of noisy equipment; <br /> • Long Pants <br /> • Work gloves and/or chemical-resistant gloves; <br /> • High visibility safety vest in the vicinity of heavy equipment; and <br /> • Sunscreen and insect repellant as necessary. <br /> Protective equipment ensembles selected for this project are anticipated to provide protection against the <br /> types and concentrations of hazardous materials that may potentially be encountered during field <br /> operations. However,no protective garment, glove or boot is resistant to all chemicals at any <br /> concentration; in fact,chemicals may continue to permeate or degrade a garment even after the source of <br /> the contamination is removed. <br /> In order to obtain optimum usage from PPE,the following procedures are to be followed by all personnel: <br /> • When using disposable coveralls, don a clean,new garment after each rest break or at the beginning <br /> of each shift. <br /> • Inspect all clothing, gloves and boots both prior to and during use for: <br /> o Imperfect seams <br /> o Non-uniform coatings <br /> o Tears <br /> o Poorly functioning closures. <br /> • Inspect reusable garments, boots and gloves both prior to and during use for: <br /> UMH:\WPROCESS\25532\WORLDENTERPRISE\WELL DESTRUCTION\ATTACHMENT C WORLD ENTERPRISES HSP TG EDITS.DOC\18-NOV-11\\ 5-3 <br />
