These records shall be made available, <br />upon request within 36 hours, to the local agency or the State Water Board . (1) Monitoring and maintenance records <br />shall be maintained for at least 36 months and include the following: (A) The date and time of all monitoring or <br />sampling; (B) The results of any visual observations; (C) The results of all sample analysis performed in the laboratory <br />or in the field, including laboratory data sheets and analysis used; (D) The logs of all readings of gauges or other <br />monitoring equipment, ground water elevations, or other test results (E) The results of inventory readings and <br />reconciliations; (F) The results of testing; and (G) The results of inspections <br />CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last <br />three years on site. <br />This is a repeat violation (Class 2) Violation. <br />CA Health and Safety Code 25292.2; CCR 2711(a)(11) 2010007 - Submitted/maintained current Cert of Financial <br />Responsibility/mech of financial assurance (TCR 11). <br />OBSERVATION: Current financial responsibility documents were not found on site. <br />REGULATION GUIDANCE: A copy of current financial responsibility documents are required to be maintained on site . <br />CORRECTIVE ACTION: Immediately obtain a copy of the facility's financial responsibility documents and maintain a <br />copy on site. <br />This is a repeat violation (Class 2) Violation. <br /> 53 <br />CA Code of Regulations 2716(a-e) 2030013 - DO inspections completed at least every 30 days and meet all <br />requirements (TCR 12). <br />1.) OBSERVATION: The following were not attached to the designated operator inspection reports: <br />- For the DO report done on 5/6/24, no work order for fuel alarm that occurred on 5/4/2024. <br />REGULATION GUIDANCE: The designated operator visual inspection report shall include: -a copy of documentation <br />demonstrating action taken in response to each compliance issue identified during the previous inspection -a copy of <br />the alarm history since the previous inspection -a copy of documentation demonstrating action taken in response to <br />each alarm since the previous inspection -a list of each alarm since the previous inspection for which there is no <br />documentation of the alarm condition and action taken in response <br />CORRECTIVE ACTION: Ensure all required documents are attached to future designated operator inspection reports. <br />----------------------------------------------------------------------------------------------------------------- <br />2.) OBSERVATION: The designated operator failed to check and note all required items on the visual inspection <br />reports.
