2. Table 02200-2 of the technical specification requires a minimum 12 -inch loose lift <br />thickness of the vegetative soil. Section 4.4 of the Final CQA Report states that "The <br />vegetative soil layer was placed in maximum 12 -inch loose lifts and compacted". This <br />is not in compliance with the technical specification. <br />The statement in the CQA Report that the "the vegetative soil layer was placed in <br />maximum 12 -inch loose lifts" was in error. In fact, the vegetative soil was placed in a <br />minimum 12 -inch loose lift and compacted In accordance with Table 02200-2. The <br />vegetative soil layer was actually placed in a 1.5 -foot (18 -inch) thick lift as recorded by <br />GLA's field technician in Daily Reports 40-47 (dates 7-16 through 7-25-13, attached). In <br />addition, photographs were taken showing a thick lift (photographs attached in daily <br />reports documenting this thickness). The daily reports were included in the Final Report <br />and where prepared by GLA's field technician during construction. <br />Attached, please find the amended Final Construction Quality Assurance Report, <br />correcting this error in Section 4.4. For ease of review, the incorrect text was struck out <br />and the revised text is in bold. <br />3. Section 02200 specification 2.05C requires that the maximum particle size shall be 2 <br />inches and specification 2.05D requires that the maximum aggregation at the surface <br />shall be 2 inches. Section 4.4 of the Final CQA Report states that "The sieve analysis <br />indicated that 100% of the material passed the 3-inche sieve, which meets the required <br />maximum particle size of 3 inches". This is not in compliance with the technical <br />specifications. <br />The 3 -inch size stated in the report was in error. The particle size of the vegetative soil <br />did not exceed the maximum 2 -inch in accordance with Section 02200 specification <br />2.05C and D. As shown on the test reports provided in Appendix C of the Final Report <br />(attached), 100% of the soil % passed the 2 -inch sieve. In fact, the actual largest particle <br />size was retained on a No. 8 sieve (2.36 mm), indicating that the soil exceeded <br />specifications. <br />Please find attached revisions to the Final CQA Report correcting this error in Section <br />4.4. The test particle size analysis report was revised to show the results in a table <br />format and are also attached. <br />4. On 16 July, the Daily Field Report #40 states that the GLA CQA technician reported <br />that one of the vegetative cover material nuclear density tests failed test <br />specifications. On 17 July 2013, the Daily Field Report #41 states that the GLA CQA <br />technician reported that the vegetative cover material nuclear density testing failed <br />13939 West Colfax Ave., Suite 220 • Lakewood, Colorado 80401 • T 303.232.3310 - F 303.232.3346 • www.geo-logic.com <br />
