Submit proof of correction/test results to the EHD. <br /> This is (Class 2)Violation. <br /> 56 Code of Fed Regulations 112.7(e), 112.8(c)(6)4030015-Aboveground containers tested or inspected for integrity <br /> based on industry standards. <br /> OBSERVATION: Facility failed to test/inspect each aboveground container for integrity based on industry standards, <br /> as discussed in the SPCC Plan: <br /> 1. On a regular schedule. <br /> 2. Inspecting each container's supports,foundations, and outside for signs of deterioration, discharges, or <br /> accumulation of oil inside diked areas. <br /> REGULATION GUIDANCE: REGULATION GUIDANCE: 112.8(c)(6)Test or inspect each aboveground container for <br /> integrity on a regular schedule and whenever you make material repairs. <br /> CORRECTIVE ACTION: Immediately conduct the necessary integrity inspections on all containers based on industry <br /> standards, on a regular schedule, inspecting each container's supports,foundations, and outside for signs of <br /> deterioration, discharges,or accumulation of oil inside diked areas and as per the SPCC plan. Maintain testing and <br /> inspection records. Submit a copy of the test/inspection results to the EHD. <br /> This is (Class 2)Violation. <br /> 71 Code of Fed Regulations 112.8(d)(5)4030031 -Vehicle traffic warned of aboveground piping or other oil transfer <br /> operations. <br /> OBSERVATION:The facility failed to warn vehicle traffic about oil transfer operations. No warnings were observed <br /> on-site at the ASTs at the facility. <br /> REGULATION GUIDANCE: (d)(5)Warn all vehicles entering the facility to be sure that no vehicle will endanger <br /> aboveground piping or other oil transfer operations. <br /> CORRECTIVE ACTION: A warning must be provided to all vehicles entering the facility to be sure that no vehicle will <br /> endanger aboveground piping or other oil transfer operations. Immediately provide a warning to all vehicle traffic <br /> entering the facility about the oil transfer operations in accordance with CFR Part 112. Provide proof of correction to <br /> the EHD. <br /> This is (Class 2)Violation. <br /> 78 Code of Fed Regulations 112.20(e)4010044-Substantial Harm Criteria certification maintained at the facility as <br /> required. <br /> OBSERVATION:The facility failed to complete and maintain (at the facility)the Substantial Harm Criteria certification <br /> form, and the owner/or operator determined that the facility could not be reasonably expected to cause substantial <br /> harm to the environment.
