A body art facility that uses single use instruments sha ll maintain record of purchase, log of procedures, names <br />of <br />practitioner and client, date of procedure, instruments used, and written evidence from the manufacturer that the <br />instruments have undergone sterilization. The record of purchase, evidence of sterilization, type and number of <br />instruments used shall be maintained for a minimum of 90 days after use. HSC 119315(f) <br /> <br />OBSERVATIONS: <br />1. Sterilization certificates were not provided for the Radical needles with lot numbers 230704 and 220508. <br /> <br />CORRECTIVE ACTIONS: <br />1. At prior inspection, operator provided EHD with a sterilization certificate. Current needles at facility still current to <br />prior sterilization certificate. EHD agreed to send copy of sterilization certificate to operator and operator will provide <br />proof of the copy being on site. <br /> <br />9. No food, drink, tobacco product, or personal effects are permitted in the procedure area. The practitioner shall <br />not eat, drink, or smoke while performing a procedure. If a client requests to eat, drink, or smoke, the procedure <br />shall be stopped and the procedure site shall be protected from possible contamination while the client leaves the <br />procedure area to eat, drink, or smoke. HSC 119309(i) <br /> <br />OBSERVATIONS: <br />1. In one of the garbage containers within the procedure there was visible soda cans and water bottles. <br />2. Mini fridge observed within the procedure area. <br />3. Snacks viewed in corner of facility. <br /> <br />CORRECTIVE ACTIONS: <br />1. Remove any food, drinks, or smoking products from the procedure area to prevent any eating, drinking or smoking <br />within the procedure area. <br />2. Remove the mini fridge from the procedure area and provide proof of correction to EHD. <br />3. Remove snacks from the procedure area of the facility. <br /> <br />18. Prior to the performance of body art, the client shall read, complete, and sign an informed consent form. HSC <br />119303(a). <br /> <br />OBSERVATIONS: <br />1. Various client consent forms were not fully completed. Missing elements included tattoo description, artist name, <br />and back half of the medical questionnaire. <br /> <br />CORRECTIVE ACTIONS: <br />1. Please assure that all client consesnt forms are included in full prior to commencing body art procedure. <br /> <br /> <br />
