The practitioner shall maintain a clean and sanitary environment. Sterile instrument packs shall be evaluated <br />before use, and if the integrity of a pack is compromised in any way, including but not limited to, being torn, <br />punctuyred, wet, or having evidence of potential moisture contamination, the instrument pack shall be discarded <br />or reprocessed before use. HSC 119309(a) & (h) <br /> <br />OBSERVATIONS: <br />1. One of the needle packs that were on site was expired. – CORRECTED ON SITE <br /> <br />32. A body art facility shall have floors and walls that are smooth, nonabsorbent, free of open holes, and washable. <br />HSC 119314(a)(2) <br /> <br />OBSERVATIONS: <br />1. Beam in the middle of the procedure area has a spot where paint is peeling. <br /> <br /> <br /> <br />CORRECTIVE ACTIONS: <br />1. Repair all of the facility defects mentioned above so as to have a smooth, nonabsorbent finish that is free of holes <br />and easily cleanable. <br /> <br />33. All counter surfaces and service trays shall have a smooth, durable, and nonabsorbent finish. HSC <br />119314(b)(6). <br /> <br />OBSERVATIONS: <br />1. Chair between the procedure bed and the sink was made of absorbable material. On the chair was a T -shirt. <br />2. Each procedure bed had a T-shirt on them. <br />3. The curtains that are covering the windows in the procedure area were made of absorbable material. <br />4. Stool left of the procedure area sink had a tear in the leather upholstery, revealing absorbable padding. <br />5. Wall to the left of the procedure area sink had a cloth banner. <br />6. Observed aprons on wall. <br /> <br />CORRECTIVE ACTIONS: <br />1. Remove all of the materials from the procedure area that are absorbable and provide proof of correction to EHD. <br /> <br />35. A body art facility shall have a written Infection Prevention Control Plan, and have records of training and shall <br />be maintained for three years and shall be available for inspection upon request of the enforcement officer. HSC <br />119313(e) <br /> <br />OBSERVATIONS: <br />1. IPCP was not provided for review on site. <br /> <br />CORRECTIVE ACTIONS: <br />1. Please provide proof that a copy of the IPCP is on site. <br /> <br /> <br />Corrections due to EHD by May 20th 2025. <br />
