San Joaquin County <br />Department of Public Works <br />County of San Joaquin Dept of Public Works IIPP Rev 2020-01-31.docx P a g e | 14 <br /> Information about chemical hazards to which employees could be exposed and <br />other hazard communication program information <br /> Availability of toilet, hand-washing, and drinking water facilities <br /> Provisions for medical services including emergency procedures <br />1.8 Reporting and Recordkeeping <br /> <br />The Department recognizes the need to keep records and report on employee <br />training and incidents related to safety. <br /> <br />1.8.1 The Department will keep to following: <br /> Training records for each employee, including: <br />o Employee Name <br />o Date and Subject of Training <br />o Training Provider <br /> The Department will use the Employee Training Record or comparable form <br /> The training records will be maintained for a period of at least five (5) years and <br />may be kept at the Division that the employee is currently assigned to <br /> A copy of the training records will be scanned and submitted electronically to the <br />Department Personnel Office for retention <br /> <br />1.8.2 Injury and Illness Records <br /> Any employee injury or illness, including the Supervisor’s Report of Accident will <br />be kept. <br /> Injury and Illness Records will be maintained for three (3) years <br /> <br />1.8.3 Safe Condition Forms <br /> Copies of the Safety Condition Forms, along with employee safety and health <br />condition concerns shall be maintained <br /> These records will be kept for three (3) years <br /> <br />1.8.4 Safety and Health Inspections <br /> Copies of the Safety and Health Inspection Forms, including any corrective <br />action(s) taken, shall be maintained <br /> These records will be kept for three (3) years <br /> <br />1.8.5 Additional Recordkeeping <br /> Additional recordkeeping requirements, depending upon the Department’s <br />operations and activities, will be kept in order to ensure compliance with Cal <br />OHSA regulations under CCR, Title 8 <br /> These records will be kept for three (3) years
