County of San Joaquin Dept of Public Works IIPP Rev 2020-01-31.docx <br /> Page 71 <br />individual shall not remove a crew lock until it has been verified that all individuals are <br />clear <br /> <br />3.6.10 Procedures Involving More Than One Shift or Person Unavailable <br /> <br />In the event that an employee has left the premises or is otherwise unavailable, <br />a supervisor may remove the locking device(s) and tag(s) after the following <br />procedure: <br /> Contact the employee(s) that has equipment locked out to find out the status of the <br />equipment when possible <br /> Clear all personnel away from the work area to a safe distance <br /> Clear away any tools and material from the work area <br /> When necessary, follow the appropriate guidelines listed the above procedures <br />to re-lockout equipment when the task is complete <br /> <br />3.6.11 Inspection and Review <br /> This lockout policy and procedure is inspected and reviewed annually <br /> Inspection includes a review of the policy under this procedure and an inventory <br />of the lockout kits to ensure all necessary lockout / tag-out materials are on hand <br /> The inspector certifies that the inspection of the procedure is completed by <br />identifying the types of machines or equipment on which this lockout policy is <br />implemented, the date of the inspection, the employees included in the <br />inspection and the person performing the inspection <br /> <br />3.6.12 Identification of Equipment Lockout <br /> Attached to this document is the list submitted regarding equipment or <br />equipment type(s) that require lockout and the appropriate lockout location(s) <br /> <br />3.7 Training <br /> <br />3.7.1 Training Requirements <br /> <br /> Only personnel trained in this procedure are authorized to perform lockouts. <br />Such employees are known as “authorized employees” and are trained on <br />hazardous energy control procedures and on the hazards related to <br />performing activities required for cleaning, repairing, servicing, setting-up <br />and adjusting prime movers, machinery and equipment. <br /> Employees in the area whose may work on equipment that has been locked <br />out or may work in the area where lockouts are performed will not perform
