These personnel, known as “affected employees” are trained in the <br />purpose and use of this lockout policy and procedure. <br /> All other employees, whose work operations may be in an area where energy <br />control procedures may be utilized, shall be instructed about the prohibition <br />relating to attempts to restart or reenergize machines or equipment which <br />are locked out or tagged out. <br /> Training is provided to all Authorized and Affected Employees when they are <br />first assigned the duties of either lockout or working on or around locked out <br />equipment. Other Employees are trained when first hired or assigned to an <br />area where lockouts are performed. Training for all three categories of <br />employees is repeated annually. <br /> <br />3.8 Reporting and Recordkeeping <br /> <br />The Department will maintain training records in two locations. <br /> <br />3.8.1 Division office training records <br /> Once a training session related to Lockout / Tag out is complete, the original <br />copy of the sign in sheet will be maintained in an area in the Division <br />Administration office <br /> The Sign in sheet will have <br />o Subject of the training event <br />o Date and start time of the event <br />o Employee Name and Current Job Title at time of training <br />o Employee Signature that employee acknowledges attending the <br />training list <br /> The sign in sheet will be on file for at least five years in the division offices or in <br />accessible storage. <br />3.8.2 Public Works Administration Office training records <br /> At the conclusion of the training event, the affected division will forward an <br />electronic copy of the training sign in sheet <br /> The electronic copy will be maintained for a period of at least 5 years <br /> <br />3.9 Policies <br />
