S A IY J O Q I I I Environmental Health Department <br /> c, Possesses a valid, current certificate of training from the manufacturer(s) of the <br /> component(s) being tested, repaired, or serviced <br /> 4, Individuals installing underground storage tank system components must possess or work <br /> under the direct and personal supervision of an individual physically present at the work site <br /> who possesses all of the following; <br /> a. A valid, current contractor's license issued by the Contractors State License Board <br /> b, A valid, current UST InstallationlRetroftting certificate issued by the International Code <br /> Council (I ) <br /> c- A valid, current certificate of training fronn the manufacturer(s) of the component(s) <br /> being installed <br /> 5. Individuals working under the direct supervision of a qualified UST installer or UST service <br /> technician must, at a minimum, be ernployed by the person with the GSLI3 license and possess <br /> all manufacturer certificates of training for the equipment on which they area working. <br /> 6- The tank owner or operator shall log into the California Environmental Deporting System <br /> ( ER ) at http-llcers.calepa-ca.govl and ensure all UST information is current. <br /> 7- All components must be certified by an independent testing organization and lalbeled as <br /> required by the organization (L)L. ULC, ANSI. API, A TM, etc ), See California Code of <br /> Regulations ( R) Title 23, Chapter 16, Table 8 for a complete list. <br /> 8- Permit expires 180 days from the date of approval. If an extension to the expiration date is <br /> required, a written request must be submitted to ustu sigov,orQ prior to the expiration date, <br /> 9. Failure to comply with the listed conditions may result in additional fees or violations. <br /> 10. Additional inspections may be conducted by the EHD.
