Please see attached message referencing WO#2486185-1.No leak detector testing took place on 1 2/212 0 2 4. The cold start monitoring system certification on 12/2/2024 was not required as this was a cold start on a dispenser. a The monitoring system certification for the 4/18/2025 sensor replacement(PN:330020-463)at S2 T1 RV Diesel FP 19.20,the notification was made on 4/22/2025 and the test report submitted on 5/1/2025. Please see attached message referencing WO#2593819-1. • For failure to submit a copy of the overfill prevention equipment testing from 3/11/2025,triennial overfill testing took place at this location on 9/19/2024. The test report was submitted on 10/4/2024. Please see attached message"Pilot 618(1501 N.Jack Tone Road)Overfill 9/19/24 for the submission email and test report. There was no overfill testing completed at this site on 3/11/2025. • For failure to provide notification for work at least 48 hours in advance of the following: a A cold start performed on 12/3/2024—no notification was required since this was a cold start on a dispenser that took place on 12/2/2024. o Relay replacement performed on 11/13/2024—notification was not required because the relay was in the dispenser. o 4/18/2025 sensor was replaced like for like using(PN:330020-463)located at S2 T1 RV Diesel FP 19.20—please see the attached email referencing WO#2593819-1 showing the after the fact notification and test report submission for the sensor replacement. a 4/18/2025 sensor 794380-303 replacement—the notification was sent on 4/14/2025. Please see attached email referencing WO#259346-1. a 4/24/2025 for a T1 Tank Diesel Probe Out alarm where the probe was replaced like for like using(PN:846397-407)located at T1—please see attached email referencing WO#2593896-1 for after the fact notification that occured on 5/5/2025. • For failure of the 87 STP to shut down and meet the requirements for annual line tightness testing exemption and the 91 in the BID TLS 350 panel was not operational due to a blown fuse on the output relay module,these issues were corrected during the monitoring system certification and all components passed testing at that time. The Work Order for this testing and tech notes is attached(Pilot 618AMC W02594613-1). • For failure to have records on site,the Designated Operator has been provided with all documentation noted as not being on site in the NOV and will ensure that any missing documentation is on site in the proper location.
