The missing alarms include: <br /> -2/19/2024 Tank 3 Motor Oil Fill fuel alarm. <br /> REGULATION GUIDANCE#1: During the inspection,the designated operator shall review the alarm history since the <br /> previous inspection, check that each alarm was documented and responded to appropriately. <br /> OBSERVATION#2:The DO failed to check and note all required items on the visual inspection reports.The incorrect <br /> and missing information included the following from the inspection reports: <br /> -Sections 7 and 8 were left blank on the 4/1/2024 inspection report <br /> -The 12/9/2024 DO report provided outdated certification expiration date for the DO <br /> -The 4/1/2024 DO report work orders did not address the previous L5 alarm <br /> REGULATION GUIDANCE#2: During the visual inspection,the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the <br /> required information on the reports. <br /> OBSERVATION#3: DO reports were not signed within the required time frame. <br /> -The 5/28/2024 DO report was signed by the owner/operator on 6/3/2024. <br /> -The owner/operator did not complete section 5 of the 4/1/2024 DO report. <br /> REGULATORY GUIDANCE#3:Within 72 hours of being provided a signed copy of the report,the owner or operator <br /> shall provide a description of each corrective action taken or to be taken and shall sign and date the report, <br /> acknowledging the identified compliance issues. <br /> CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the <br /> required time frame in the future. <br /> This is a repeat violation (Class 2) Violation. <br /> 58 CA Code of Regulations 2715(c)(3), 2715(c)(4)2010015- Copies of Facility Employee Training Certificates maintained. <br /> OBSERVATION: The Facility Employee Training Certificate dated 1/11/2024 contained incorrect"Initial Training <br /> Dates". Initial training dates were said to have been completed before the date of assuming responsibility by facility <br /> contacts. Previous training certificates supported this statement. <br /> REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if <br /> after October 1, 2018, or within 30 days of hire if prior to October 1, 2018.
