The contractor is responsible for alerting the Permit Department of prior expiration <br /> dates of all enclosed Permits. <br /> 1. UST Retrofit—No Fee Permit—UST related work that requires no fee and no permit application <br /> All conditions below must be met: <br /> a) Provide at least 48-hour advance written notification to ust@sjgov.org for repair and testing. <br /> b)The component must be tested after repair/installation to verify functionality. <br /> c)The service company must provide the following documentation to the tank owner/operator for <br /> on-site review by an inspector during the annual UST inspection: <br /> 1) Description of part(s)installed (parts list,cut sheets,etc.) <br /> 2) Proof of current ICC and manufacturer certifications for individual performing the work <br /> 3)Testing documentation to verify functionality of the component after repair <br /> d)All repaired components must be tested within 30 days of repairs. <br />
