SA QQQ N Environmental Health Department <br /> COUNTY <br /> Greotness draws here. <br /> Underground Storage Tank (UST) Permit Application Guidance <br /> Permit applications can be located at www.sigov.org/department/envhealth/forms. All permit <br /> requests should be submitted to Environmental Health Department (EHD) by email to ust@sjgov.org <br /> or by mail to 1868 E. Hazelton Ave., Stockton, CA 95205 or in person. The submittal should include: <br /> • an 'Application Form' <br /> • the appropriate permit application, with all required attachments <br /> • payment or proof of payment <br /> Permits must be approved by the EHD prior to beginning work for non-emergency repairs. Permit <br /> applications for emergency repairs must be submitted within one (1) business day of the repair. <br /> Emergency repairs are unplanned repairs relating to monitoring and release detection only. <br /> 'Failure to Obtain a Permit' penalty: The 'Failure to Obtain a Permit' penalty is an additional 100%of <br /> the UST Retrofit- Full Permit fee. If a required permit is not obtained, or all conditions of the No Fee <br /> Permit are not met, a Full Permit must be obtained with the penalty applied ($1,032 total). <br /> TYPES OF PERMITS—Refer to pages 2 & 3 for a list of UST work and the required permit type <br /> 1. UST Installation Permit—$3440 <br /> EHD written approval needed—comply with all conditions on the approval letter. <br /> 2. UST Closure Permit—Temporary Closure: $516, Removal or Closure in Place:$516 per tank <br /> EHD written approval needed—comply with all conditions on the approval letter. <br /> 3. UST Retrofit—Full Permit—$516 (Note: Tank Lining is$516 per tank <br /> EHD written approval needed—comply with all conditions on the approval letter. <br /> 4. UST Retrofit—Minor Plan Check Permit—$172 <br /> EHD written approval needed—comply with all conditions on the approval letter. <br /> S. UST Retrofit—No Fee Permit— UST related work that requires no fee and no permit application <br /> All conditions below must be met: <br /> a) Provide at least 48-hour advance written notification to ust@sjgov.org for repair and testing. <br /> b) The component must be tested after repair/installation to verify functionality. <br /> c) The service company must provide the following documentation to the tank owner/operator for <br /> on-site review by an inspector during the annual UST inspection: <br /> 1) Description of part(s) installed (parts list,cut sheets, etc.) <br /> 2) Proof of current ICC and manufacturer certifications for individual performing the work <br /> 3) Testing documentation to verify functionality of the component after repair <br /> d) All repaired components must be tested within 30 days of repairs. <br /> 6. No Permit Needed—no fee and no permit application <br /> No END notification or approval needed. <br /> 1868 E.
