All <br /> technicians performing tests and certifications are individually certified by the equipment <br /> manufacturers and hold applicable International Code Council (ICC) certificates. <br /> CP22-UST Full Inspection Report Dated January 11, 2024: <br /> CGRS's submitted the following Scope of Work& applied for a repair permit to address the following <br /> open items in the 2024 Inspection Report on January 16, 2025: <br /> 1. Item # 104, 120, & 311- Provide and install one (1)Veeder Root 409 sensor in the diesel/E85 <br /> tank annular. The sensor should be a 345 not a 409, plans and site map edited for correct <br /> sensor models numbers <br /> 2. Item #214- Obtain permit for the Veeder Root 208 sensor installed in the turbine sump <br /> 3. Item #303- Pull back test boot in the E85 turbine sump completed during 2025 monitor cert <br /> 4. Item # 306- Check all sumps for water and pump out as necessary completed during 2025 <br /> monitor cert <br /> 5. Item #317- Obtain permit for the diesel line leak detect and replace with new line leak <br /> detector not needed MLD passed 2025 testing <br /> 6. Item # 318- Obtain permit for the failed 91 product spill bucket and replace with like for like <br /> spill bucket not needed spill bucket passed 2025 testing <br /> 7. Item # 106- Provide an updated site map that meets CUPA's requirements <br /> CP22-UST Full Inspection Report Dated January 25, 2025: <br /> 1. Item #5- Provide an update site map with fills and turbine sumps correctly identified. See <br /> attached <br /> 2. Item # 10-Contractor has failed to send copy of the annual monitoring certification that was <br /> witnessed by Inspector Alberto Fernandez. <br /> 3. Item # 12- Provide copy of the SB989 report, see attached report <br /> 4. Item # 14- Provide copy of the OPI Certification report, see attached report <br /> 5. Item # 30- e85 test boots need to be pulled back, see attached report <br /> 6. Item # 54- e85 tank annular sensor is a 345 and is e85 compatible, see attached cut sheet <br /> 7.
