CGRS SOLUTIONS DELIVERED update the parts list on page 3 to include manufacturer and models for all regulated parts being used. I want to make sure I approve you for the right stuff. Page 3 of 6: • install 208 in turbine sump (Veeder-Root) -208 diesel sensor was installed by previous contractor during the 2024 inspection without a finalized permit • install diesel mechanical leak detector(Vaporless) • install 91 spill bucket (OPW) -diesel spill bucket was tested and passed the 2025 monitor certification without breaking ground Written letter: • install Veeder-Root 409 sensor in diesel/E85 tank annular -plans should read 345 sensor is installed, see attached site map • Veeder-Root 208 sensor in turbine sump (diesel turbine sump) • pull back test boot in E85 turbine sump completed at monitor cert on 1-24-25 • replace diesel leak detector—not needed leak recent • replace 91 spill bucket (like for like) Spill bucket passed testing without replacement Map: new map attached • install Veeder-Root 345 sensor in diesel/E85 tank annular -to compound this issue the original sensor was the correct 345 sensor and passed the 2025 inspection • obtain permit for Veeder-Root 208 replacement in diesel turbine sump with revised SOW • pull back test boot on E85 product line in turbine sump -completed at 2025 inspection • install new VMI LD-2000 leak detector on diesel turbine - not needed leak detector passed testing without replacement • install new OPW diesel spill bucket—not needed 2. The leak detector is listed as the VMI LD-2000, but the cut sheet submitted is for the VMI 99 LD- 2000. Which part will be used? -VMI -LD 2000 is correct 3. No cut sheet was submitted for the diesel/E85 annular sensor. Please submit a cut sheet and E85 compatibility information. 345 cut sheet attached 4. The VMI certification submitted was for operation of the tester box. No certification was submitted for installation of the leak detector. Please submit a VMI installer certification for Richard Thomas, or full certifications for another service technician. Garret Warren performed the leak detector certification during the 2025 inspection, ICC& manufacture certs attached CGRS's Revised Scope of Work-replace the 208 e85 turbine sump sensor with a 323 sensor&submit permit application for the diesel 208 turbine sump sensor, see attached SOW&Site Plan.
