OBSERVATION:The designated operator failed to: <br /> -Check and note all required items on the visual inspection reports. Multiple DO reports had Section 8 with multiple <br /> answers for the same question. (Ex.YES and N/A).The 1/25/24 DO report had Section 13 left blank, Section 9 did <br /> not list all Spill Containers on site <br /> - Document all the alarms from the attached alarm history on the 1/25/24 DO report and failed to check that they were <br /> responded to appropriately.Alarms occurred on 1/23/24 were not noted on the report. <br /> - Note the correct test dates on the 1/25/24 DO report. The monitoring system certification, spill container and overfill <br /> had 1/23/2023 listed but should be 1/23/2024. <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing <br /> and maintenance for the UST system have been completed and shall check all required items on the report <br /> CORRECTIVE ACTION: Ensure all designated operator inspection reports include all required information. <br /> 2.OBSERVATION: The 12/27/2024 DO report did not have an owner/operator response in Section 5 to the L6 sensor in <br /> alarm compliance issue in Section 3.The 6/12/2024 DO report had a prefilled signature date for the owner/operator. <br /> REGULATORY GUIDANCE: Provide a description of each corrective action taken for any compliance issues during the <br /> inspection on the copy of the DO Report signed by the designated UST operator; and Within 72 hours of being <br /> provided a signed copy of the Designated Underground Storage Tank Operator Visual Inspection Report,"the owner or <br /> operator shall: Sign and date the report, acknowledging the results of the inspection. <br /> CORRECTIVE ACTION: Ensure owner/operators are including corrective action statements in section 5 to compliance <br /> issues and are signing/dating the DO reports. <br /> This is a repeat violation (Class 2) Violation. <br /> Overall Inspection Comments: <br /> Routine UST Inspection <br /> An inspection checklist was provided to the facility operator on the day of inspection, January 14, 2025.
