A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of <br />practitioner and client, date of procedure, instruments used, and written evidence from the manufacture that the <br />instruments have undergone sterilization. The record of purchase, evidence of sterilization, type and number of <br />instruments used shall be maintained for a minimum of 90 days after use. HSC 119315(f) <br /> <br />OBSERVATIONS: <br />1. No sterile certificate was provided for the Inkin Revo needle with the lot number 112301. <br />2. No sterile certificate was provided for the Cheyenne needles with lot number 2024-04-044. <br />3. No sterile certificate was provided for precision needles with the lot numbers 240310 (16 gauge) and 241015 (18 gauge <br />and 14 gauge) <br /> <br />CORRECTIVE ACTIONS: <br />1. Please provide the necessary sterilization certificates for the needles mentioned above. <br /> <br /> <br />11. Procedure areas in the body art facilty shall be equipped with a sink that is supplied with hot and cold water, <br />containerized liquid soap, and single-use paper towels from a wall mounted touchless dispenser that is accessible to <br />the practitioner. HSC 119314(b)(4) <br /> <br />OBSERVATIONS: <br />1. The right most sink, in the large tattoo room did not have cold running water available. <br /> <br />CORRECTIVE ACTIONS: <br />1. Please repair the sink to have cold and hot running water and su bmit proof of corrections to EHD. <br /> <br />25. All solid surfaces and objects in the procedure area and the decontaminations and sterilization area that have come <br />into contact with the client shall be cleaned and decontaminated after each use by the application of a disinfectant , <br />used according to the manufacturer’s directions. HSC 119303(b) <br /> <br />OBSERVATIONS: <br />1. Infection Prevention Control Plan (IPCP) indicated that disinfectants use a wet contact time of 1-5 minutes. Madacide <br />observed in the small tattoo room was noted to have a 6 minute wet contact time <br /> <br />CORRECTIVE ACTIONS: <br />1. Update the IPCP to indicate correct wet contact times used for the disinfectancts that are being used within the facility. <br />2. Have staff trained on IPCP modifications and send proof of training to EHD. <br /> <br /> <br /> <br /> <br />
