Many of the procedure chairs within the facility had duct tape that was being used to cover the holes for the arm rest <br />chairs. <br /> <br />CORRECTIVE ACTIONS: <br />1. Cover the holes for the arm rests with the proper caps or install the arm rests so that the holes are no longer exposed. <br />Send proof of correction to EHD. <br /> <br /> <br /> <br /> <br />CORRECTIONS DUE TO EHD BY 6/11/25 <br /> <br /> <br /> <br /> <br />Notes: <br />1. Report is emailed to portcityink209@aol.com. <br />2. Client consent forms were reviewed from February 28, 2025 – May 26, 2025 for tattoo, and February 28,2025 – May 25, <br />2025 for piercing. <br />3. Needles that were located in the purple room that did not have lot numbers nor brand association were removed from <br />the procedure area and will no longer be in use. <br />4. In the smaller tattoo room, there was a bottle opener mounted on the doorway. Because there is no food or drink <br />allowed in the procedure areas, this item should be removed to prevent the access of any drinks within the procedure <br />area. <br />5. Caviwipes were not noted on the IPCP. Noted to operator to add Caviwipes to the list of disinfectants used in the IPCP. <br />Send updated copy of IPCP to EHD. <br />6. Permit for Cintia Landin was left at the practitioner’s station. Valid BBP was posted at the practitioner’s station. <br />7. Metal composition verification was made by reviewing jewelry purchase website and order history documents. <br />8. Facility opened on 6/26/24. Waste hauler is Barnett Medical Services. Waste manifest viewed that was dated 2/10/25. <br />9. A valid BBP was posted at each practitioner currently working at the facility. <br />10. Autoclave for facility produced by The Pelton and Crane Company, is a Model S, with a serial number A7-09495. <br /> <br /> <br /> <br />
