HSC 117960 – LQG shall file MWMP with the EA. <br /> <br />OBSERVATION: <br />The facility does not have a Medical Waste Management Plan (MWMP). <br /> <br />CORRECTIVE ACTION: <br />A large quantity generator required to register with the enforcement agency shall file with the enforcement agency a MWMP. Submit <br />a copy of the facility’s MWMP that meets the requirements of 117820. A copy of the San Joaquin County Environmental Health <br />Dept’s MWMP template was provided to Frank at the time of the inspection. Complete and return the form to the EHD. <br /> <br />13. HSC 117975 – LQG shall maintain treatment and tracking documents for 2 years. <br /> <br />OBSERVATION: <br />1. Per Frank Cervantes (Mainteannce Director), their biohazardous waste is picked up every 2 weeks. The following tracking <br />records were missing: all of 2023, February 2024-May 2024, and the 2nd tracking record for January, June, July, August, <br />September, and October. <br />2. Tracking records do not contain treatment facility information. <br /> <br />CORRECTIVE ACTION: <br />1. A large quantity medical waste generator shall maintain for a minimum of two years individual treatment records and the track ing <br />documents for all untreated medical waste shipped offsite for treatment. Provide the missing tracking records. <br />2. Ensure that the missing tracking records provided for item #1 contain treatment facility information. <br /> <br />24. HSC 118275 – Medical, biohazardous, sharps, trace chemo, pathology, or pharmaceutical waste shall be segretated and <br />contained pursuant to this section. The container shall be propery labeled. <br /> <br />OBSERVATION: <br />1. A 5 gallon bucket containing pharmaceutical waste (IV with antibiotics) was observed in nurse station 3 & 4 without proper <br />labeling. (Photo 5) <br />2. The pharmaceutical waste containers in nurse station 1 & 2 is full and left open. The pharmaceutical container in the sub-acute <br />nurse station is also full. (Photo 1 & 7) <br />3. Mixed waste was observed inside the sharps container on the respiratory therapy cart. (Photo 6) <br />4. Biohazardous containers in the accumulation area have faded labels on the lids. (Photo 3) <br /> <br />CORRECTIVE ACTION: <br />1. Pharmaceutical containers shall be labeled with the words “HIGH HEAT” or “INCINERATION ONLY,” or with another label <br />approved by the department, on the lid and sides, so as to be visible from any lateral direction . Submit photos of the correction. <br />Provide training and submit training records. <br />2. Replace full medical waste containers to ensure that employees have the means to containerize/store medical waste at the point <br />of generation. Keep containers closed when not in use. Submit photos of corrections. Provide training and submit training records. <br />3. Medical waste shall be contained separately from other waste at the point of origin. Provide training and submit training records. <br />4. Containers shall be labeled with the words “Biohazardous Waste” or with the international biohazard symbol and the word <br />“BIOHAZARD” on the lid and sides so as to be visible from any lateral direction.
