Unfortunately, I was unable to close any violations with the document submitted and all <br /> violations remain open. No statement or supporting documentation was provided for violations 3 or 4. <br /> Violations 5, 11 and 12 could not be closed because the updated CERS submittal was not accepted due <br /> to unresolved issues identified during the inspection. Please see feedback on CERS and original <br /> inspection report for details on how to correct these violations. For violation 14, training records must be <br /> submitted to me for review, having them stored on site is not sufficient to close the violation. <br /> I've attached the original inspection report again for your convenience.
