This permit shall be effective as of the date shown below. <br />FOR San Joaquin County Environmental Health Department <br />Dated: <br />-5- <br />TREATMENT SYSTEMS <br />16 The Pacific Bell UE-17L Water System shall have the EHD approval for any <br />installation or alterations of a treatment system prior to the Pacific Bell UE-17L Water <br />System serving water that is being treated by the altered water system. A procedural <br />violation will be issued to the Pacific Bell UE-17L Water System, if the system is <br />serving water that is being treated by an installed or altered treatment system that the <br />EHD has not reviewed and approved for installation. <br />This permit supersedes all previous domestic water supply permits issued for this public water <br />system and shall remain in effect unless and until it is amended, revised, reissued, or declared <br />to be null and void by the San Joaquin County Environmental Health Department. This <br />permit is non-transferable. Should the Pacific Bell UE-17L Water System undergo a change <br />of ownership, the new owner must apply for and receive a new domestic water supply permit. <br />Any change in the source of water for the water system, any modification or the method oi <br />treatment as described in the Permit Report, or any addition of distribution system storage <br />reservoirs shall not be made unless an application for such change is submitted to the San <br />Joaquin County Environmental Health Department. <br />Adrienne Ellsaesser, REHS <br />Program Coordinator
