9 <br />-4- <br />The EHD will only accept Routine and Repeat samples that have chlorine residuals <br />reported on the laboratory report above 0.05 parts per million (ppm), if the Pacific Bell <br />UE-17L Water System has prior approval from the EHD to use a chlorination feed <br />system. <br />15 The Pacific Bell UE-17L Water System shall provide the EHD with a copy of the valid <br />CDO license from the State Department of Health Services of the personnel who will be <br />making operational decisions concerning the Pacific Bell UE-17L Water System. The <br />EHD has Kevin DeJesus on file as the CDO. <br />CERTIFIED OPERATORS <br />14 The Pacific Bell UE-17L Water System shall utilize only personnel who are certified in <br />accordance with Title 22, Section 7107, California Code of Regulations, as a Grade 1 <br />Certified Distribution Operator (CDO) to make decisions addressing the following <br />operational activities: make alterations, repairs, and additions to the water lines; <br />disinfect, flush, drain, clean existing water lines; pull, rehabilitate and disinfect the water <br />well; stand-by emergency responses, maintain or adjust the flow and pressure, including <br />fire flows: investigate water quality problems, determine and control the proper chemical <br />dosage rates for the wellhead disinfection and distribution system. <br />OUT OF SERVICE WELLS <br />13 The Pacific Bell UE-17L Water System shall obtain an Out of Service Well Permit from <br />EHD for any well that is no longer used and is inactive. The Out of Service Well Permit <br />is valid for one year. A well that is inactive for longer than one year is defined as an <br />abandoned well, per the California Well Standards and the San Joaquin County Well <br />Standards. All abandoned wells shall be destroyed under permit and inspection from <br />the EHD, unless the Pacific Bell UE-17L Water System demonstrates to the EHD that <br />the well will become active and the well is not a conduit for contaminants to migrate and <br />degrade the ground water. <br />10 All four Repeat sample results shall be enumerated and chlorine residuals of each <br />sample reported on the laboratory report. Any Repeat sample result submitted that is not <br />enumerated or fails to report the chlorine residual reported or will not be accepted by <br />the EHD. <br />11 All five Routine samples collected in a month, following the month of a positive Routine <br />sample result, shall be enumerated and chlorine residuals reported of each sample <br />reported. Any one of the five Routine sample results submitted that is not enumerated <br />or fails to report the chlorine residual will not be accepted by the EHD. <br />12 All Routine and Repeat sample results that are found to be positive for the presence of <br />coliform bacteria, whether they are acceptable or not acceptable, will be used by the <br />EHD to determine if the Pacific Bell UE-17L Water System is in compliance with the <br />drinking water standards for bacteria
