STATE OF CALIFORNIA <br />DOMESTIC WATER SUPPLY PERMIT <br />Issued To <br />Pacific Bell Corp. <br />PWS No. #3901086 <br />April 2, 2015DATE:PERMIT NUMBER PT 0006238 <br />WHEREAS: <br />i. <br />2. <br />3. <br />4. <br />- 1 - <br />By <br />San Joaquin County Environmental Health Department <br />This public water system is known as the Pacific Bell-UE17L Water System whose <br />headquarters is located at 2300 E Eight Mile Road, Stockton CA. <br />Pacific Bell Environmental Management submitted an application dated March <br />25, 1997, to San Joaquin County Environmental Health Department to operate a <br />public water system. The application was submitted in accordance with California <br />Health and Safety Code, Section 116525. <br />The legal owner of the Pacific Bell UE17L Water System is Pacific Bell Corp. <br />Pacific Bell Corp., therefore, is responsible for compliance with all statutory and <br />regulatory drinking water requirements and the conditions set forth in this permit. <br />The public water system for which the permit application has been submitted is as <br />described briefly below: <br />This is a Non-Transient Non-community water system that serves 100 <br />employees.
