Broadway Ave. <br /> REGULATION GUIDANCE:Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval. <br /> This is (Minor)Violation. <br /> 11 CA Health and Safety Code 25505(a)(2) 1010005-Site Map with all required content electronically submitted. <br /> OBSERVATION:The site map is incomplete/inaccurate: <br /> -The fire extinguisher noted at the fuel canopy was not observed during the inspection <br /> -The location of the fire extinguisher in the store was observed on the west of door instead of east <br /> -There was an additional fire extinguisher observed in the southwest corner of the store that was not noted on the map <br /> -The location of propane was not noted on the map <br /> REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, <br /> evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br /> present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map <br /> requirements the governing body of the unified program agency finds necessary. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is(Minor)Violation. <br /> 14 CA Health and Safety Code 25505(a)(4) 1020002-Provided initial and annual training and maintained training records <br /> for a minimum of three years. <br /> OBSERVATION: HMBP training records for the past 3 years were not observed on site. The only employee on site, R. <br /> Yakobi stated he has not yet received any formal training.At the time of inspection, he was been employed for 1 <br /> week. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive <br /> initial and annual refresher training.All employees shall be trained in safety procedures in the event of a release or <br /> threatened release of a hazardous material, including, but not limited to,familiarity with the following plans and <br /> procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the <br /> unified program agency. (B) Procedures for the mitigation of a release or threatened release to minimize any potential <br /> harm or damage to persons, property, or the environment. (C)Evacuation plans and procedures, including immediate <br /> notice,for the business site.
