Environmental Health Department <br />AMENDED <br />3/4/2025 <br />CP22 - HMBP Inspection Report <br />Date: <br />February 12, 2025 <br /> Facility Address: <br /> 3440 E MAIN ST, STOCKTON <br /> Facility Name: <br /> NASHIR MAIN ST INC. <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />This is (Class 2) Violation. <br />CA Health and Safety Code 25505(a)(1) 1010004 - Complete and accurate Hazardous Materials Inventory information <br />electronically submitted. <br />OBSERVATION: The following was found on site that has not been reported to CERS: <br />- Approximately 315 lbs (74 gals) of propane was observed on site on the side of the building along S. Broadway Ave. <br />REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the <br />facility's business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval. <br />This is (Minor) Violation. <br /> 10 <br />CA Health and Safety Code 25505(a)(2) 1010005 - Site Map with all required content electronically submitted. <br />OBSERVATION: The site map is incomplete/inaccurate: <br />- The fire extinguisher noted at the fuel canopy was not observed during the inspection <br />- The location of the fire extinguisher in the store was observed on the west of door instead of east <br />- There was an additional fire extinguisher observed in the southwest corner of the store that was not noted on the map <br />- The location of propane was not noted on the map <br />REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, <br />evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br />present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs , as well as additional map <br />requirements the governing body of the unified program agency finds necessary. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is (Minor) Violation. <br /> 11 <br />CA Health and Safety Code 25505(a)(4) 1020002 - Provided initial and annual training and maintained training records <br />for a minimum of three years. <br />OBSERVATION: HMBP training records for the past 3 years were not observed on site. The only employee on site, R. <br />Yakobi stated he has not yet received any formal training . At the time of inspection, he was been employed for 1 <br />week. <br />REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive <br />initial and annual refresher training. All employees shall be trained in safety procedures in the event of a release or <br />threatened release of a hazardous material, including, but not limited to, familiarity with the following plans and <br />procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the <br />unified program agency. (B) Procedures for the mitigation of a release or threatened release to minimize any potential <br />harm or damage to persons, property, or the environment. (C) Evacuation plans and procedures, including immediate <br />notice, for the business site.
