OBSERVATION:The following was found on site that has not been reported to CERS: -Approximately 315 lbs(74 gals)of propane was observed on site on the side of the building along S. Broadway Ave. REGULATION GUIDANCE:Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval. This is (Minor)Violation. 11 CA Health and Safety Code 25505(a)(2) 1010005-Site Map with all required content electronically submitted. OBSERVATION:The site map is incomplete/inaccurate: -The fire extinguisher noted at the fuel canopy was not observed during the inspection -The location of the fire extinguisher in the store was observed on the west of door instead of east -There was an additional fire extinguisher observed in the southwest corner of the store that was not noted on the map -The location of propane was not noted on the map REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map requirements the governing body of the unified program agency finds necessary. CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is(Minor)Violation. 14 CA Health and Safety Code 25505(a)(4) 1020002-Provided initial and annual training and maintained training records for a minimum of three years. OBSERVATION: HMBP training records for the past 3 years were not observed on site. The only employee on site, R. Yakobi stated he has not yet received any formal training.At the time of inspection, he was been employed for 1 week. REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training.All employees shall be trained in safety procedures in the event of a release or threatened release of a hazardous material, including, but not limited to,familiarity with the following plans and procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified program agency. (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment. (C)Evacuation plans and procedures, including immediate notice,for the business site. This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years.
