If a client requests to eat, drink, or smoke, the procedure shall be <br /> stopped and the procedure site shall be protected from possible contamination while the client leaves the procedure <br /> area to eat, drink, or smoke. Prior to the performance of body art, the client shall read, complete, and sign an informed consent form that shall <br /> include, but not be limited to all of information within this section. HSC 119303 <br /> OBSERVATIONS: <br /> 1. Client consent forms were not available for review during the inspection. <br /> 2. On the aftercare instruction sheet, restrictions on physical activity that were missing were gardening and contact with <br /> animals. <br /> CORRECTIVE ACTIONS: <br /> 1. Please maintain client consent forms onsite for enforcment agency to review during inspection. <br /> 2. Update the aftercare instructions to include the elements above. Send proof of corrections to EHD. <br /> 32. A body art facility shall have floors and walls that are smooth, nonabsorbent, free of open holes, and washable. <br /> HSC 119314(a)(2) <br /> OBSERVATIONS: <br /> 1. The baseboard within the procedure area had spots where the paint is chipping. <br /> CORRECTIVE ACTIONS: <br /> 1. Please make any necessary repairs mentioned above and send proof of correction to EHD. <br /> 33. A body art facility shall have all counter surfaces and service trays that have a smooth, durable, and nonabsorbent <br /> finish. HSC 119314(b)(6). <br /> OBSERVATIONS: <br /> 1.
