Inspection is emailed to davidpropes76(a�yahoo.com and to sabrinaramos3(a�yahoo.com <br /> 2. Republic sharps is the waste hauler. 1 Waste tracking manifest was reviewed. Facility opened September 2023. Noted <br /> to operator that pick ups need to be made at a minimum once per year. <br /> 3. Client consent forms were being kept electronically. Only files reviewed were from 2023. Operator will try to look for files <br /> and provide to EHD for review. In the case that client consent forms are not available for review, a statement must be <br /> made to EHD in regards to the maintenance of the client consent forms. <br /> 4. Madacide is used as disinfectant. Correct contact time given by practitioner. <br /> 5. IPCP was reviewed on site. <br /> 6. Operator noted that they may move to another space within the same building, and add piercing as a service. I noted to <br /> operator that another consultation would be needed and filling out application paper work is necessary. <br /> 7. Above Training was used for BBP training. As of this date it is not an approved course. Updated list for BBP training <br /> providers was given to operator. Above training for BBP was accepted this time only due to operator not having access <br /> to updated list.
