DEUEL VOCATIONAL INSTITUTION <br />No. 157 - OPERATIONAL PROCEDURE - Medical Waste Management Plan <br />V. Methods -Continued <br />F. Medical Waste Contractor <br />Stericycle, Incorporated, 2333 Waukegan Road, Suite 300, Bannockburn, <br />IL, 60015 is the authorized contractor for the removal, transportation and <br />treatment of medical waste generated by Deuel Vocational Institution. <br />The local designated Stericycle facility is at 4135 W. Swift Avenue, <br />Fresno, CA 93722. Telephone number (559) 275-0994. <br />G. At the conclusion of each service call, the Medical Waste Contractor shall: <br />Remove all medical wastes according to the scheduled timeframe <br />specified under "Transportation of Medical Waste to Disposal <br />Area". <br />2. Clean and disinfect each storage bin, locker and/or area; as <br />needed, maintain designated medical waste sites free of medical <br />waste spills and any debris related to the pickup of medical waste. <br />3. Inspect all designated medical waste containers to ensure <br />compliance with Health Care Services Division, Chapter 9, <br />commencing with Section 118275. The institution will conduct <br />periodic inspection of the storage bins and lockers and its <br />immediate area. Contractor shall immediately correct all unsanitary <br />conditions or deficiencies as noted by institution inspections. <br />4. Within fifteen (15) calendar days after a medical waste pick up, a <br />copy of the manifest with the disposal site information section <br />completed shall be furnished to the CHSA I, or designee. Invoices <br />will be processed upon receipt by the CHSA I or designee and will <br />include a copy of the treatment, storage and disposal (TSD) facility <br />manifest, establishing acceptance of the waste at the disposal site, <br />and the Certificate of Disposal from the ultimate disposal facility. <br />5. Contractor shall provide an official scale, approved by the State <br />Department of Food and Agriculture Measurement Standards <br />Division, to weigh medical waste. Contractor is responsible for <br />weighing all medical waste at the institution prior to transport. <br />Contractor shall report the total pounds picked up to the institution <br />liaison prior to transport. <br />0 <br />
