A large quantity medical waste generator shall maintain, for a minimum of two years, the tracking document for all <br />untreated medical waste shipped offsite for treatment. Documentation shall be made available to the enforcement agency <br />onsite. HSC 117975 (a) & (b) <br />OBSERVATIONS: <br />1. Pharmaceutical tracking documents were not available for view. Dates needed to review from 9/11/23 – 3/18/25. <br />2. Medical waste manifests were not available for between the following dates: <br />a. 12/18/23 – 1/5/24 <br />b. 5/20/24 – 5/28/24 <br />c. 8/26/24 – 9/9/24 <br />d. 1/6/25 – 1/14/25 <br />3. Date discrepancy on the medical waste manifest document dated 2/11/25. Generator dated the document 12/11/24. <br />CORRECTIVE ACTIONS: <br />1. Provide all necessary documents for the pharmaceutical waste to EHD and maintain pharmaceutical waste tracking <br />documents onsite. <br />2. Provide the tracking documents for the medical waste to EHD and maintain all medical waste tracking documents onsite. <br />3. Provide training to employees to properly fill out and maintain tracking documents. Provide proof of training to EHD. <br /> <br />24. Medical, biohazardous, sharps, trace chemo, pathology, or pharmaceutical waste shall be segregated and contained <br />pursuant to this section. The container shall be properly labeled. HSC 118275 <br />OBSERVATIONS: <br />1. The sharps container in the south station medical cart contained commingled waste (solid waste mixed with sharps <br />waste). <br />2. The pharmaceutical containers located in the north and east medical rooms, were not labeled properly. Bin contained IV <br />bags. Training document submitted to EHD notes that flush syringes are placed into the white bins used for <br />pharmaceutical wastes. This denotes that sharps waste are also put into the pharmaceutical bins. <br />3. Over flowing container in designated accumulation room. Per operator, the bin is used for medical waste. Bin did not have <br />“High Heat” or “Incineration Only” labels. Bin also contains sharps attached to the vial bags. Bin did have the biohazard <br />symbol. <br />4. At the north station nurses carts, biohazard containers were not labeled properly. <br />5. Multiple nurses carts throughout the facility had clear plastic bags lining the biohazard container attached to the cart. <br />CORRECTIVE ACTIONS: <br />1. Train staff on medical waste segregation and submit training records to EHD. <br />2. Due to sharps and pharmaceuticals being placed within the same bin, please label the bin with “HIGH HEAT” <br />or “INCINERATION ONLY” and with “sharps waste”, “BIOHAZARD” or with the international biohazard symbol. Please <br />maintain the bins with a tight fitting lid.
