SAP/QAPP Addendum <br /> Lodi-125 N Stockton <br /> Final <br /> March 24,2025 <br /> 9.0 SAMPLE IDENTIFICATION, DOCUMENTATION, AND SHIPMENT <br /> This section addresses field notes,sample labeling,and sample chain-of-custody forms and custody seals. <br /> 9.1 Field Notes <br /> The following subsections discuss field logbooks and photographs. <br /> 9.1.1 Field Logbooks <br /> All field logbook activities will accord with Toeroek Team SOP#017-2,Recording of Notes in Field <br /> Logbook,in Appendix C. <br /> 9.1.2 Photographs <br /> Photographs will be taken at sampling locations and at other areas of interest on the Site.The photographs <br /> will serve to verify information entered in the field logbook. For each photograph taken,the following <br /> information will be written in the logbook: <br /> • Time,date,location,and weather conditions; <br /> • Description of the subject photographed,including the cardinal direction the photograph was taken; <br /> and <br /> • Name of the person taking the photograph. <br /> 9.2 Sample Labeling <br /> All samples collected will be labeled in a clear and precise way for proper identification in the field and for <br /> tracking in the laboratory. Sample ID nomenclatures are listed in Site-Specific QAPP Worksheet#18.The <br /> samples will have preassigned,identifiable,and unique numbers.At a minimum, sample labels will display the <br /> following information: sample location,date of collection,analytical parameter(s),and method of <br /> preservation. Every sample,including samples collected at a single location but going to separate laboratories, <br /> will be assigned a unique sample number per Site-Specific QAPP Worksheet#18. <br /> 9.3 Sample Chain-of-Custody Forms and Custody Seals <br /> All sample custody procedures will accord with QAPP Worksheet#27 in the Final Programmatic QAPP <br /> Revision 2 (EPA R9/QA DCN: FY22BNFD227) (Toeroek Team 2022). <br /> 59 <br />
