The document provided by the facility is for a "proposed" project <br />and does not show materials of all conveyances and containers. <br />-The facility failed to provide documentation demonstrating that the waste wash water conveyances and containers are <br />visually inspected twice per year to ensure that waste wash water is not leaking into the underlying soil as required. The <br />facility provided documentation demonstrating that the visual inspections are conducted once per year. <br />-The visual inspection records submitted by the facility show that wastewater pit #1 failed hydrostatic testing on 9/28/24 <br />and 3/1/25. On 9/28/24, the pit lost 12 inches within 24 hours and then an additional 4.5 inches, totaling 16.5 inches lost. <br />On 3/1/25, the pit lost 13 inches within 24 hours and then an additional 4.5 inches, totaling 17.5 inches lost. <br />-The contingency plan submitted by the facility is limited to spill response and does not adequately address procedures <br />for handling both onsite and offsite emergencies for the materials. <br />(Continued in Overall Inspection Comments section at bottom of report) <br />This is a Repeat (Class 1) Violation. <br />1868 E.
