HSC 119315 -Invoices and log kept for disposable, pre-sterilized equipment <br /> OBSERVATIONS: <br /> Sterilization certificates were not available for Garrett Hardin's Kwadron cartridges with the lot#24069 or Tattoo Express (XXX <br /> Tattoo) needle cartridges with the lot#2026-03. A sterilization certificate was not available for Gabriel Lane's Kwadron cartridges <br /> with the lot#1206. <br /> CORRECTIVE ACTIONS: <br /> A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner and client, <br /> date of procedure, instruments used, and written evidence from the manufacture that the instruments have undergone <br /> sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a <br /> minimum of 90 days after use. Provide EHD with sterilization certificates for the needles mentioned above. <br /> 9. HSC 119309 - No eating,drinking or smoking -clean clothes <br /> OBSERVATIONS: <br /> Drinks were observed near Garrett Hardin's workstation and Mr. Morato was seen eating while walking through the procedure <br /> area. <br /> CORRECTIVE ACTIONS: <br /> No food, drink, tobacco product, or personal effects are permitted in the procedure area. Refrain from eating or drinking in the <br /> procedure area. <br /> 13. HSC 119307 -Bloodborne Pathogen Training <br /> OBSERVATIONS: <br /> A Bloodborne Pathogen Training Certificate was not available for Garbriel Lane. —(corrected) <br /> CORRECTIVE ACTION: <br /> Practitioner shall provide evidence of a completed, EHD approved, OSHA Bloodborne Pathogen Training consistent with section <br /> 119307. <br /> 19. HSC 119303, 119308-Appropriate aftercare instructions given to client <br /> OBSERVATIONS: <br /> The aftercare instructions did not include all of the items from Form B. <br /> CORRECTIVE ACTIONS: <br /> At the completion of the procedure, the practitioner shall provide post procedure instructions that include all elements listed in <br /> Form B. Include the following on the aftercare instructions and provide a copy to the EHD: <br /> -Signs and symptoms of infection. <br /> - Indications when to seek medical care. <br /> Reinspection on/about: A reinspection fee of$172 per hour may be charged.
