HSC 119314- Floors and walls clean and in good repair, adequate light <br /> OBSERVATIONS: <br /> Holes observed in the wall near Gabriel Lane's workstation. <br /> CORRECTIVE ACTIONS: <br /> A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. Repair <br /> and repaint the wall. Provide EHD with photographic evidence of corrections. <br /> 33. HSC 119314-Workstation,surfaces, including chairs,armrests, etc. in good repair <br /> OBSERVATIONS: <br /> 1. Jazzmin Martinez's and Gabriel Lane's procedure chairs are torn. <br /> 2. Absorbent decorations were observed throughout the facility, absorbent jacket observed at Garrett Hardin's workstation <br /> (corrected), absorbent jacket and chair observed at Mario Pina Morales'workstation (corrected). <br /> CORRECTIVE ACTIONS: <br /> All surfaces and objects in the procedure area, including chairs, armrests, tables, countertops, and trays, shall be in good repair <br /> and capable of being disinfected. Repair or replace the chairs mentioned above and provide EHD photographic evidence of <br /> correction. <br /> 37. HSC 119312- Permits obtained and available <br /> OBSERVATIONS: <br /> 1. EHD was not notified that Phillip Mendoza or Garrett Fransen were no longer practitioners at the facility. <br /> 2. EHD was informed by staff that Anthony Bottom will be practicing at the facility soon. Anthony Bottom's business card and <br /> social media indicate he's already practicing at the facility. <br /> CORRECTIVE ACTIONS: <br /> 1. A body art facility shall notify the LEA in writing within 30 days of resignation or termination of a practitioner. <br /> 2. A person shall not perform body art without a valid registration and a permitted facility.Anthony Bottom is to reactivate his <br /> permit IMMEDIATELY and pay all past due fees. <br /> Correct All Violations and Submit Evidence of Corrections to the EHD <br /> by April 28, 2025. <br /> Reinspection on/about: A reinspection fee of$172 per hour may be charged.
