Permittee shall conduct surface testing using an organic vapor analyzer, flame ionization detector, or other portable <br />monitor meeting the specifications provided in the condition that references Section 63.1960(d) in this permit. Surface <br />testing shall be conducted at all cover penetrations. Any reading of 500 ppm or more above background at any location <br />shall be recorded as a monitored exceedance. An exceedance is not a violation of the surface methane operational <br />standard if the following actions are taken: (1) The owner or operator shall determine the latitude and longitude <br />coordinates of each exceedance using an instrument with an accuracy of at least 4 meters. The coordinates shall be in <br />decimal degrees with at least five decimal places, (2) For each exceedance, cover maintenance or adjustments shall be <br />made to the vacuum of the adjacent wells to increase the gas collection in the vicinity of the exceedance. The location <br />shall be re-monitored within 10 days of detecting the exceedance, (3) If the re-monitoring of the location shows a <br />methane concentration less than 500 ppm methane above background concentration, the location shall be re-monitored <br />1 month from the initial exceedance, and (4) If the re-monitoring shows a second exceedance, additional corrective <br />action shall be taken and the location shall be re-monitored again within 10 days of the second exceedance. If the re- <br />monitoring shows a third exceedance, a new well or other collection device shall be installed within 120 days of the <br />initial exceedance. An alternative remedy to the exceedance, such as upgrading the blower, header pipes or control <br />device, and a corresponding timeline for installation may be submitted to the APCO for approval. [40 CFR 60.8, 40 <br />CFR 63.1958(d), and 40 CFR 63.1960(c)] Federally Enforceable Through Title V Permit <br />57. Permittee shall implement a program to monitor for cover integrity and implement cover repairs, as necessary, on a <br />monthly basis. [40 CFR 63.1960(c)(5)] Federally Enforceable Through Title V Permit <br />58. Permittee shall comply with the following specifications and procedures for surface methane monitoring: (1) The <br />portable analyzer shall meet the instrument specifications provided in section 6 of EPA Method 21 of appendix A of <br />40 CFR Part 60, except that "methane" replaces all references to "VOC," (2) The calibration gas shall be methane, <br />diluted to a nominal concentration of 500 ppm in air, (3) The instrument evaluation procedures of section 8.1 of EPA <br />Method 21 of appendix A of 40 CFR Part 60 shall be used, and (4) The calibration procedures provided in sections 8 <br />and 10 of EPA Method 21 of appendix A of 40 CFR Part 60 shall be followed immediately before commencing a <br />surface monitoring survey. [40 CFR 63.1960(d)] Federally Enforceable Through Title V Permit <br />59. The collection and control system shall comply with all applicable provisions of 40 CFR 63 Subpart AAAA at all <br />times including startup, shutdown, and malfunction, and the SSM requirements of the General Provisions of 40 CFR <br />63, Subpart A do not apply. During periods of startup, shutdown, or malfunction the permittee shall comply with the <br />work practice requirement specified in the condition that references 40 CFR 63.1958(e) in this permit in lieu of <br />complying with the conditions that reference provisions of 40 CFR 63.1960 in this permit. [40 CFR 63.1930(b) and <br />63.1960(e)(2)] Federally Enforceable Through Title V Permit <br />60. "Startup" is defined as the activity of starting or attempting to start operation of the collection and control system, <br />including all measurements, manipulations, adjustments, attempts to restore operation, as well as testing to verify <br />proper operation of the collection and control system. Unsuccessful attempts to restart the collection and control <br />system shall not be considered an individual shutdown. The owner/operator shall minimize emissions during startup. <br />[District Rule 2201] Federally Enforceable Through Title V Permit <br />61. "Shutdown" is defined as the cessation of operation of the collection and control system. [District Rule 2201] Federally <br />Enforceable Through Title V Permit <br />62. "Malfunction" means any sudden, infrequent, and not reasonably preventable failure of air pollution control <br />equipment, process equipment, or a process to operate in a normal or usual manner. Failures that are caused in part by <br />poor maintenance or careless operation are not failures. [40 CFR 60.2] Federally Enforceable Through Title V Permit <br />63. "Maintenance" is defined as work performed on the collection and/or control system in order to ensure continued <br />compliance with all applicable Rules and Regulations, and also to prevent failures and malfunctions. The <br />owner/operator shall minimize emissions during maintenance periods. [District Rule 2201] Federally Enforceable <br />Through Title V Permit <br />64. Permittee shall install a sampling port and a thermometer, other temperature measuring device, or an access port for <br />temperature measurements at each wellhead.
