<br />72. For corrective action that is required by the condition that references 40 CFR 63.1960(a)(3) and conditions that
<br />reference 40 CFR 62.16720(a)(4) in this permit and is not completed within 60 days after the initial exceedance,
<br />permittee shall submit a notification to the APCO as soon as practicable, but no later than 75 days after the initial
<br />temperature exceedance. [40 CFR 62.16724(k) and 40 CFR 63.1981(j)(1)] Federally Enforceable Through Title V

<br />73. For corrective action that is required by the condition that references 40 CFR 63.1960(a)(3) and conditions that
<br />reference 40 CFR 62.16720(a)(4) in this permit and is expected to take longer than 120 days after the initial
<br />exceedance to complete, permittee shall submit the root cause analysis, corrective action analysis, and corresponding
<br />implementation timeline to the APCO as soon as practicable, but no later than 75 days after the initial temperature

<br />74. Permittee shall keep at least 5 years up-to-date, readily accessible on-site records of the design capacity report that
<br />triggered 40 CFR 63.1959(b), the current amount of solid waste in-place, and the year-by-year waste acceptance rate.

<br />75. Permittee shall keep at least 5 years up-to-date, readily accessible records for the life of the control system of the
<br />maximum expected gas generation flow rate as calculated in 40 CFR 63.1960(a)(1) and the density of wells, horizontal
<br />collectors, surface collectors, or other gas extraction devices as determined necessary to achieve comprehensive
<br />control of surface gas emissions by a professional engineer. The following issues shall be addressed in the design:
<br />Depths of refuse, refuse gas generation rates and flow characteristics, cover properties, gas system expandability,
<br />leachate and condensate management, accessibility, compatibility with filling operations, integration with closure end
<br />use, air intrusion control, corrosion resistance, fill settlement, resistance to the refuse decomposition heat, ability to
<br />isolate individual components or sections for repair or troubleshooting without shutting down entire collection system,
<br />landfill gas migration issues and augmentation of the collection system through the use of active or passive systems at
<br />the landfill perimeter or exterior. [40 CFR 63.1983(b)(1)] Federally Enforceable Through Title V Permit
<br />76. Permittee shall keep at least 5 years up-to-date, readily accessible records of the average combustion temperature in the
<br />enclosed flare(s) measured at least every 15 minutes, averaged over the same time period of the performance test, and
<br />the percent reduction of NMOC achieved by the enclosed flare(s) as measured during the initial performance test.
<br />Records of subsequent tests and monitoring shall be maintained for a minimum of 5 years. Records of the enclosed
<br />flare(s) vendor specifications shall be maintained until removal. [40 CFR 63.1983(b)(2)] Federally Enforceable

<br />77. Permittee shall keep at least 5 years up-to-date, readily accessible continuous records of the following: (1) All 3-hour
<br />periods of operation of the enclosed flare(s) during which the average combustion temperature was more than 28
<br />degrees Celsius below the average combustion temperature at which compliance with the NMOC reduction rate or
<br />concentration was determined, (2) The indication of flow to the control system and the indication of bypass flow or
<br />records of monthly inspections ensuring the bypass valve is maintained in the closed position, (3) Periods when the
<br />collection system or the control device are not operating, (4) The date, time and duration of each startup and/or
<br />shutdown period, (5) For each instance where the collection and control system fails to meet an applicable standard,
<br />the date, time, duration, a list of affected sources or equipment, and actions taken to minimize emissions according to
<br />40 CFR 63.1955(c) and any corrective actions taken to return the affected unit to its normal operation, and (6) For the
<br />temperature monitoring device and the device that measure flow to the flares or bypass, the written procedures for
<br />initial and any subsequent calibration of the devices, determination and adjustment of the calibration drift of the
<br />devices, preventative maintenance of the devices (including spare parts), data recording, calculations, and reporting,
<br />accuracy audit procedures (including sampling and analysis methods), and program for corrective action for
<br />malfunctioning devices. [40 CFR 63.1983(c)] Federally Enforceable Through Title V Permit
<br />78. Permittee shall keep for at least 5 years up-to-date, readily accessible records of all collection and control system
<br />exceedances of the surface methane operational standard, the operational standard for wellhead temperature of landfill
<br />gas, and wellhead pressure operational standard. For each exceedance, the operator shall record the reading in the
<br />subsequent month, whether or not the second reading is an exceedance, and the location of each exceedance. [40 CFR
