Mr. Jeff Wong <br />San Joaquin County Environmental Health Department <br />May 8, 2023 <br />Page 2 <br />The site is currently surrounded by an ornamental iron security fence and the ground surface is covered <br />by a combination of bare soil and old concrete/asphalt. The remnant of a fuel dispenser island is located <br />along the northeast corner of the property, adjacent to the station building's foundation. The property is <br />accessed by gates located along the northern and the eastern property lines. <br />Project Background <br />A summary of the Site's history and environmental assessment and remediation activities performed at <br />the Site is included in Enclosure B. <br />Proposed Well Destructions Scope of Work <br />To complete closure of the CVRWQCB's leaking underground storage tank (UST) case, the CVRWQCB <br />has requested proper destruction of the Site's remaining 16 monitoring and remediation wells (MW -1 <br />through MW -8, MW -10, MW -11, MW -1B, MW -5B, MW -6B; air sparge well AS -1, and soil vapor <br />extraction wells SVE-IA/B and SVE-2A/B. The locations of the wells are illustrated on Figure 2 and a <br />summary of the well construction details are included on Table 1. Copies of the well construction logs <br />for all the wells are included in Enclosure C. <br />The wells will be destroyed in accordance with the State of California Department of Water Resources <br />(DWR) Well Standards (Bulletins 74-81 and 74-90) and the San Joaquin County Well Standards. The <br />proposed scope of work for the well destructions will consist of the following tasks: <br />Underground Utility Location: USA North will be notified (811 or 800-227-2600) a minimum of 2 <br />working days in advance of the drilling activities so that USA North's member subscribers can mark the <br />locations of their utilities. In addition, RDM will also subcontract a private utility locator to search and <br />identify utilities whose owners are not member subscribers of USA. <br />RDM may require the driller to hand clear around any well whose destruction may pose a risk of damaging <br />underground utilities. Hand clearing would be done to a depth of 5 feet below surface grade (fbgs) using <br />either an air knife or hand auger to identify any potential subsurface utilities that may have been installed <br />after the wells were installed. <br />Site Health and Safety Plan: A site specific health and safety plan (SHSP) will be prepared by RDM. <br />The SHSP will identify potential site hazards and appropriate personal protective equipment (PPE), <br />provide emergency contact information and directions to nearest hospital or treatment facility. A copy <br />will be maintained on-site during fieldwork. <br />Right -of -Entry Agreements: The following monitoring wells are located off-site on the adjacent <br />property and Huntbrook has an existing right -of -entry agreement for them: <br />• MW -4 — 3200 East Main Street (APN 157-110-510-000) <br />• MW -6 and MW -6B — 3139 East Main Street (APN 157-082-270-000) <br />• MW -7 — 3130 East Main Street (APN 157-104-020-000) <br />The off-site property owners will be notified a minimum of two weeks in advance of the proposed well <br />destructions. <br />Encroachment Permits: The following wells are in the City of Stockton right-of-way and will require <br />an encroachment permit(s): <br />
