But in brief, <br /> you would need to provide to the EHD the following: <br /> 1. Completed Master File Record, especially the billable party information.This will also act as a <br /> service request to review your work plan/proposal <br /> 2. Completed Site Mitigation Well/boring permit application for each separate parcel (APN). Please <br /> be aware well boring permits are only issued to C-57 licensed holder per California requirement, <br /> and therefore requires either their signature on the permit application, or a person authorized by <br /> the C-57 licensed holder on the attached License Contractor Declaration form (see bottom of the <br /> form). <br /> 3. Work Plan complete with map of where the borings/well are to be installed, and construction <br /> diagram of any proposed monitoring wells. Must include a little bit of background/history/future <br /> developmental goal(can be very brief: who, what,when,where, why.A short paragraph or two will <br /> work, more if required), and the proposed analytical testing. <br /> 4. If this work is required by a lead agency(i.e. EPA, RWQCB, DTSC, etc.),their approval <br /> documentation for this proposed work <br /> 5. Access agreement(or encroachement, revocable permits if in city/county/government Right of <br /> Way) if the work is being done on a property that does not belong to your client <br /> 6. Fees. <br /> -$468 for the plan check fee which covers 3 hours (if it's a large or complicated plan, it may <br /> require $780 which covers 5 hours) <br /> - Boring Permit fees averages $468 ($156/hour) per parcel(each parcel requires a separate <br /> permit), which buys 3 hours of inspection time. Please be aware that if the inspection <br /> goes over the 3 hours which includes drive time, the time over the 3 hours will be billed to <br /> the billable party. Hence there are occasions where more than 3 hours is submitted to <br /> avoid being billed at a later time. Please feel free to contact me for variations of the fees <br /> required. <br /> -Well Permit fees: $156 per well (not parcel) per parcel, but the fees maxes out at 5 wells per <br /> parcel. <br /> 2 <br />
