Also included is the following supporting documentation: • San Joaquin County Master File of Record Form • RDM's monitoring well installation work plan; • CVRWQCB's work plan approval letter;and, • RDM's site health and safety plan The fees for this permit of$760($456 work plan review fee and$304 for the well installation permit) will be paid by RDM using San Joaquin County's on-line payment system. It is understood the$304 fee for the well installation permit is based on two wells at$152 each which includes 2 hours of inspection time,and that any additional inspection time will be billed at rate of$152 per hour. RDM is currently in the process of obtaining an encroachment permit from the City of Stockton for the installation these two proposed monitoring wells since both wells are in the City right-of-way. We expect to have the encroachment permit in 2-3 weeks.A copy of the encroachment permit will be forwarded to you as soon as we receive it. I understand that you can not issue the monitoring well installation permit until you have received a copy of the City encroachment permit.
