Also included is the following supporting documentation: <br /> • San Joaquin County Master File of Record Form <br /> • RDM's monitoring well installation work plan; <br /> • CVRWQCB's work plan approval letter;and, <br /> • RDM's site health and safety plan <br /> The fees for this permit of$760($456 work plan review fee and$304 for the well installation permit) <br /> will be paid by RDM using San Joaquin County's on-line payment system. It is understood the$304 <br /> fee for the well installation permit is based on two wells at$152 each which includes 2 hours of <br /> inspection time,and that any additional inspection time will be billed at rate of$152 per hour. <br /> RDM is currently in the process of obtaining an encroachment permit from the City of Stockton for <br /> the installation these two proposed monitoring wells since both wells are in the City right-of-way. We <br /> expect to have the encroachment permit in 2-3 weeks.A copy of the encroachment permit will be <br /> forwarded to you as soon as we receive it. I understand that you can not issue the monitoring well <br /> installation permit until you have received a copy of the City encroachment permit. <br /> The wells are scheduled to be installed on November 12-13,2019,so your expedient review is greatly <br /> appreciated. <br /> If you have any questions,please call me at(916)749-9222. <br /> Cost Effective Solutions <br />
