The UST response plan on site did not have contact information for the responsible person(s) REGULATION GUIDANCE: §2690. (a)An application for an operating permit must be submitted to the Unified Program Agency by the owner or operator. The application must include the following information, to the extent such information is known to the applicant: (8) Monitoring program in accordance with section 2650 including the facility: (A) Monitoring plan; (B) Monitoring site plan, and; (C) Response plan. § 2691. (d)A paper or electronic copy of the permit and all conditions and attachments, including the monitoring program, must be readily accessible at the facility. CORRECTIVE ACTION: Immediately obtain the most current copies of the UST response plan retain it on site. This is a Repeat(Class 2)Violation. 3 23 CCR 16 2613(a)(3) and (b), 2650(a), (b), and (e) 2011011 - Facility has submitted and maintains a complete and accurate UST Monitoring Plan OBSERVATION: Owner/Operator failed to submit and/or maintain a complete and accurate UST Monitoring Plan. 1)The monitoring plan lists that the UDC Panel Manufacturer is Veeder-Root and the UDC Panel Model # is TLS-350. But the UDC sensors are Veeder Root standalone sensors which are not connected to Veeder Root Panels. The monitoring panel section for the UDCs should be blank. 2)The 87 Pipe Monitoring Leak Sensor Manufacturer and Sensor Model is listed as Veeder Root 0794380-208. On site was observed as a PMP 63228. REGULATION GUIDANCE: § 2650(b) Owners or operators must submit a monitoring plan for approval by the Unified Program Agency which establishes: (2)The release detection methods and release detection equipment, identified by name and model, to be used for performing the monitoring CORRECTIVE ACTION: 1) Submit a complete and accurate UST Monitoring Plan and maintain copy onsite for review. 2) The sensor was replaced during the inspection from a PMP 63228 to a Veeder Root 208. This now matches the monitoring plan in CERS. This is a Repeat(Class 2)Violation. 4 23 CCR 16 2613(a)(4) and (b), 2650(a), (c), and (e) 2011012 - Facility has submitted and maintains a monitoring site plan OBSERVATION: Owner/Operator failed to submit and maintain a monitoring site plan for approval by the UPA. The monitoring site plan used two maps to meet requirements. Per State Water Board only one map should be uploaded to meet requirements of§ 2650(c). REGULATION GUDIANCE: § 2650(c) Monitoring Site Plan —Owners or operators must submit a monitoring site plan for approval by the Unified Program Agency. The monitoring site plan must include, but is not limited to, the following: (1)A scaled diagram indicating the layout of the tank(s)and piping to the extent known, including containment sumps; (2) Locations of all release detection equipment; and (3) If applicable, each vacuum, pressure, or hydrostatic interstitial monitoring zone. CORRECTIVE ACTION: Submit and maintain a monitoring site plan and ensure a copy is maintained on site. This is a (Minor)Violation.
