OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. The <br /> repair monitoring certificate test results for the S7 vac sensor replaced on 7/18/2024 were not on site. <br /> We have no record of receiving the test results in question from the provider, Walton Engineering, who is no <br /> longer in business. Please advise how to proceed. <br /> OBSERVATION:According to the work order dated 9/4/24 on site, the S2 "Rul Sumps Vac" sensor was replaced on <br /> 8/15/24 and a permit was not obtained from the EHD. <br /> S2 was not replaced on 8/15/24.
