S A N J O A Q U I N Environmental Health Department <br /> COUNTY <br /> 5. Any individual performing installations or repairs of UST system components that do not require <br /> excavation or backfill must: <br /> • Possess or be employed by a person who possesses all applicable licenses and <br /> certifications required by the CSLB; and <br /> • Possess or work under the direct and personal supervision of a service technician physically <br /> present at the work site at all times while work is being performed who possesses a current <br /> California UST Service Technician certificate issued by the ICC; and <br /> • Possess a valid, current certificate of training issued by the manufacturer(s) of the <br /> component(s) being installed or repaired or the test equipment being used. <br /> 6. Secondary containment must be tested for tightness upon installation and at least once every <br /> 36 months by a properly qualified service technician. <br /> 7. The tank owner or operator must submit completed test results for secondary <br /> containment testing within 30 days of performing the tests. These results can be <br /> submitted by email to ust(a-)-sjgov.org or by uploading to CERS. <br /> 8. The tank owner or operator shall log into the California Environmental Reporting System <br /> (CERS) at http://cers.calepa.ca.gov/ and ensure all UST information is current. <br /> 9. This permit expires 180 days from the date of approval. If an extension to the expiration date is <br /> required, a written request must be submitted to ust(o)-sigov.org prior to the expiration date. <br /> 10. Failure to comply with the listed conditions may result in additional fees or violations. <br /> 11. The permit application included payment for 1 hour of time. Additional inspections and review <br /> may be required to verify that this UST retrofit/repair work complies with all applicable laws and <br /> regulations. Any EHD staff time spent beyond the prepaid amount will be billed to the applicant <br /> at the current EHD hourly rate ($179). <br /> 12. Additional inspections may be conducted by the EHD.
