Possess or be employed by a person who possesses all applicable licenses and <br /> certifications required by the Contractors State License Board (CSLB); and <br /> b. Possess or work under the direct and personal supervision of an individual physically present <br /> at the work site at all times while work is being performed who possesses a current UST <br /> Installation/Retrofitting certificate issued by the International Code Council (ICC); and <br /> c. Possess a valid, current certificate of training issued by the manufacturer(s) of the <br /> component(s) being installed or repaired. <br /> 5. Any individual performing installations or repairs of UST system components that do not require <br /> excavation or backfill must: <br /> • Possess or be employed by a person who possesses all applicable licenses and <br /> certifications required by the CSLB; and <br /> • Possess or work under the direct and personal supervision of a service technician physically <br /> present at the work site at all times while work is being performed who possesses a current <br /> California UST Service Technician certificate issued by the ICC; and <br /> • Possess a valid, current certificate of training issued by the manufacturer(s) of the <br /> component(s) being installed or repaired or the test equipment being used. <br /> 6. Release detection equipment must be tested upon installation and at least once every 12 <br /> months for operating condition, programming, and calibration by a properly qualified service <br /> technician. <br /> 7. The tank owner or operator must submit completed test results for release detection <br /> equipment testing within 30 days of performing the tests. These results can be <br /> submitted by email to ust(a)-sigov.org or by uploading to CERS. <br /> 8. The tank owner or operator shall log into the California Environmental Reporting System <br /> (CERS) at http://cers.calepa.ca.,qov/ and ensure all UST information is current. <br /> 9. This permit expires 180 days from the date of approval. If an extension to the expiration date is <br /> required, a written request must be submitted to ust(.sigov.org prior to the expiration date. <br /> 10. Failure to comply with the listed conditions may result in additional fees or violations. <br /> 11. Additional inspections may be conducted by the EHD.
